慢性自发性荨麻疹与部分生化指标的相关性分析

【摘要】:目的1、分析慢性自发性荨麻疹患者的病情特点;2、探讨慢性自发性荨麻疹的发生与部分生化指标的联系;3、探讨部分生化指标对慢性自发性荨麻疹严重程度的影响。方法收集2015年4月至2016年8月于首都医科大学宣武医院皮肤性病科就诊的慢性自发性荨麻疹患者,记录患者的一般情况、对每位患者进行UAS评分及BMI测算,抽取患者空腹状态下外周静脉血进行生化指标检测,包括尿酸(uric acid,UA)、血糖(serum glucose,GLU)、甘油三酯(triglyceride,TG)、总胆固醇(total cholesterol,TCH)、低密度脂蛋白(low density lipoprotein,LDL-C),同时对每位患者进行血清总IgE检测。同期收集于首都医科大学宣武医院进行健康体检者作为对照组,收集其相关生化检测结果。用Microsoft Excel 2007对数据进行整理,用SPSS 22.0处理并分析数据。结果1、72例慢性自发性荨麻疹患者中,男性24例,女性48例,平均年龄(43.5±13.0)岁。33例对照组中,男性17例,女性16例,平均年龄(46.8±18.0)岁。两组的年龄、性别均无统计学差异(p0.05)。2、72例慢性自发性荨麻疹患者中,27.8%的患者风团发作频率为每天多次,50%的患者风团发作频率为每天一次,22.2%的患者风团发作频率为多天一次。3、72例慢性自发性荨麻疹患者中,33.3%的患者风团持续时间小于1h,31.9%的患者风团持续时间为1-4h,8.3%的患者风团持续时间为4-8h,9.7%的患者风团持续时间为8-16h,13.8%的患者风团持续时间为16-24h,2.8%的患者风团持续时间大于24h。4、72例慢性自发性荨麻疹患者中,2.8%的患者UAS评分为2分,15.3%的患者UAS评分为3分,26.4%的患者UAS评分为4分,31.9%的患者UAS评分为5分,23.6%的患者UAS评分为6分。5、72例慢性自发性荨麻疹患者中,2.8%的患者体重指数小于18.5,50%的患者体重指数在18.5-23.9之间,34.7%的患者体重指数在24.0-27.9之间,12.5%的患者体重指数大于28.0。6、72例慢性自发性荨麻疹患者与33例健康对照者相比,患者组TG升高率(23.6%)明显高于对照组(6.1%),其差异具有统计学意义(p=0.030),患者组与对照组的UA升高率、GLU升高率、TCH升高率、LDL-C升高率均无统计学差异(p0.05)。7、13例UAS评分1-3分者与59例UAS评分4-6分者相比,UA升高率、GLU升高率、TG升高率、TCH升高率、LDL-C升高率均无统计学差异(p0.05)。8、13例UAS评分1-3分者与59例UAS评分4-6分者相比,BMI异常率无统计学差异(p=1)。9、13例UAS评分1-3分者与59例UAS评分4-6分者相比,血清总IgE升高率具有统计学差异(p=0.048)。结论1、慢性自发性荨麻疹的发生可能受到甘油三酯升高的影响;2、慢性荨麻疹严重程度可能与血清总IgE呈正相关。
[Abstract]:Objective 1. To analyze the characteristics of patients with chronic spontaneous urticaria, (2) to explore the relationship between the occurrence of chronic spontaneous urticaria and some biochemical indexes, and (3) to explore the influence of some biochemical indexes on the severity of chronic spontaneous urticaria. Methods from April 2015 to August 2016, the patients with chronic spontaneous urticaria were collected from the Department of Dermatology, Xuanwu Hospital, Capital Medical University. The general situation of the patients was recorded, and the UAS score and BMI of each patient were calculated. Peripheral venous blood samples were taken from the patients on an empty stomach for biochemical examination, including uric acid (uric acid,UA), blood glucose (serum glucose,GLU), triglyceride (triglyceride,TG), total cholesterol (total cholesterol,TCH), low density lipoprotein (low density lipoprotein, (LDL (low density lipoprotein,). At the same time, the serum total IgE of each patient was detected by LDL-C. At the same time, the healthy physical examiners were collected from Xuanwu Hospital of Capital Medical University as the control group, and the results of biochemical tests were collected. Microsoft Excel 2007 was used to organize the data, and SPSS 22.0 was used to process and analyze the data. Results 1. Among the 72 patients with chronic spontaneous urticaria, there were 24 males and 48 females with an average age of (43.5 卤13.0) years. In the control group, there were 17 males and 16 females with an average age of (46.8 卤18.0) years. There was no significant difference in age and sex between the two groups (p0.05). 2. In 72 patients with chronic spontaneous urticaria, 27.8% of the patients had multiple episodes of wind mass a day, and 50% of the patients had a frequency of wind mass attack once a day, and there was no significant difference in age and sex between the two groups (p0.05). 22.2% of the patients had a multiple-day attack frequency. 3. Of 72 patients with chronic spontaneous urticaria, 33.3% had a wind mass duration of less than 1 hour, and 31.9% of the patients had a wind mass duration of 1-4 hours. 8.3% of the patients had a duration of 4-8 hours, 9.7% had a duration of 8-16 hours, and 13.8% had a duration of 16-24 hours, while 8.3% had a duration of 4-8 h, 9.7% had a duration of 8-16 h and 13.8% had a duration of 16-24 h. Of the 72 patients with chronic spontaneous urticaria, 2.8% had a UAS score of 2, 15.3% had a UAS score of 3, and 26.4% had a UAS score of 4. The UAS score was 5 in 31.9% of the patients, and 23.5% in 23.6% of the patients with chronic spontaneous urticaria. The body mass index (BMI) of 34.7% of the patients was between 24.0 and 27.9, 12.5% of the patients was more than 28.0.6%, and 72 of the patients with chronic spontaneous urticaria were compared with 33 healthy controls. The increase rate of TG in the patient group (23.6%) was significantly higher than that in the control group (6.1%), and the difference was statistically significant (p < 0.030). The increase rate of UA, GLU and TCH in the patient group and the control group was significantly higher than that in the control group (P < 0.05). There was no statistical difference in LDL-C elevation rate (p0.05). 13 cases with UAS score of 1-3 were compared with 59 cases with UAS score of 4-6 points, UA rising rate, GLU rising rate, TG rising rate and TCH rising rate were higher than those of 59 cases with UAS score of 4 and 6 points, and there was no statistical difference between them (p0.05). There was no significant difference in LDL-C elevation rate (p0.05). 13 cases with UAS score of 1-3 were compared with 59 cases with UAS score of 4-6 points. There was no significant difference in the abnormal rate of BMI between the 13 cases with UAS score of 1-3 and the 59 cases with UAS score of 4 and 6, and the increase rate of total IgE in serum was statistically different (p < 0. 048) compared with that of 59 cases with UAS score of 4-3 points (p < 0. 048). Conclusion 1. The occurrence of chronic spontaneous urticaria may be affected by the increase of triglyceride, and the severity of chronic urticaria may be positively correlated with serum total IgE.
