寻常型银屑病患者血清IL-23和IL-27水平的表达

【摘要】:目的:银屑病是一种以上皮细胞增生、分化失控和大量T细胞浸润为特征的慢性复发性炎症性皮肤病。银屑病的发病机理目前尚不十分清楚,受遗传、代谢异常及神经心理等诸多因素的影响,免疫功能异常在其发病中具有重要作用。研究显示辅助性T细胞亚群及其细胞因子在自身免疫病的发病及治疗中均具有重要的作用。白介素(interleukin,IL)为一大类主要由活化的淋巴细胞及树突状细胞产生的细胞因子,具有多种生物学作用。大量研究表明IL-12家族各成员以及同其他细胞因子之间存在着相互协同、相互拮抗的网络,不仅在细胞内感染及炎症过程中起着重要的调节作用,而且与银屑病、多发性硬化症、Crohn,s病等多种临床疾病的发病密切相关。目前为止,IL-12家族共有4个成员:IL-12、IL-23、IL-27和IL-35。其中IL-12、IL-23和IL-27主要由树突状细胞、单核-巨噬细胞和EBV转化的B淋巴母细胞产生,IL-35主要由Treg产生。此外,也可由EBV转化的B细胞和胎盘产生。 IL-23由p19和IL-12中的p40两个亚基组成,它们之间以二硫键相结合。主要由活化的单核细胞、巨噬细胞(Mφ)和树突状细胞(DC)产生。近年有关IL-23与银屑病皮损的相关性的研究较多,且主要研究方向为银屑病皮损处IL-23表达水平与银屑病严重程度的相关性研究以及IL-23与银屑病皮损动物模型建立的相关研究。有关银屑病患者血清中IL-23水平与疾病的相关性研究的报道较少。IL-27由EBI3和P28两个亚基组成,其中EBI3与IL-12的p40亚基同源, p28与IL-12的p35亚基同源。IL-27主要产生于抗原呈递细胞(单核细胞、巨噬细胞、树突状细胞),在单核细胞衍生的树突状细胞、活化的单核细胞和活化的巨噬细胞中水平最高。国内尚未见有关银屑病患者血清中IL-27水平与疾病严重程度的相关性研究报道。 本实验通过检测寻常型银屑病患者外周血血清中的IL-23和IL-27水平与正常对照组比较,研究血清中IL-23、IL-27表达水平与寻常型银屑病发病的相关性及与病情发展的关系,进一步探讨银屑病的发病机制,为银屑病的预防和治疗提供新思路。 方法:病例组:寻常型银屑病患者29例,均来自2010年6月至2010年12月河北医科大学第二医院皮肤科门诊,符合银屑病的诊断标准。排除红斑狼疮、天疱疮、皮肌炎等自身免疫性疾病以及其它严重系统性疾病,且三个月内未应用过免疫抑制剂或皮质类固醇激素等影响自身免疫功能的药物。并根据病情将病例组分为两组:静止期组8例,进展期组21例。对照组:11例健康者,来自河北医科大学第二医院健康工作人员,均无免疫性疾病及家族史,经t检验,性别、年龄与病例组相比无显著差异。由同一工作人员对病例组患者进行PASI评分,采集对照组和病例组静脉血各约2ml,分离血清,采用ELISA法检测血清中IL-23和IL-27表达水平的值,所得数据用SPSS17.0软件进行统计学处理,因测得的IL-23的数据满足正态性和方差齐性,故用完全随机设计的两均数比较的统计学方法;银屑病患者组中的进展期组和静止期IL-23表达水平满足正态性和方差齐性,故也可采用上述方法进行统计学处理。病例组和正常对照组所测得的IL-27的数据均不具有正态性,用两个独立样本的非参数检验分析两组间差别。检验水准均取P0.05有统计学意义。如病例组和正常对照组所得结果具有显著性差异,采用两变量的Pearson相关分析对病例组IL-23和IL-27表达水平与相应的PASI评分水平进行相关分析,以P0.05作为有统计学意义。 结果: 1血清中IL-23浓度测定结果(单位pg/ml):寻常型银屑病组平均值为(51.556±7.081),明显高于正常对照组(17.862±6.639) (t=13.657, P0.05),差异有统计学意义,病例组中进展期组与静止期组无显著性差异(t=0.627,P0.05)。病例组IL-23与临床PASI评分经过直线相关性分析有正相关关系(r=0.629,P0.05)。 2血清中IL-27浓度测定结果(单位pg/ml)):病例组与对照组的数值分布均不满足正态性,应用非参数检验,两组无统计学差别。U值为128.000, P=0.336,P0.05。 结论: 1寻常型银屑病患者血清中IL-23表达水平高于正常对照组,表明IL-23的高表达为银屑病患者免疫异常表现之一,证实了寻常型银屑病的发病与T细胞免疫失衡有关。 2寻常型银屑病血清中IL-23的表达水平在进展期组与静止期组无显著差异,说明IL-23的表达水平与银屑病的进展无明确关系,不能作为反应疾病进展情况的指标。 3寻常型银屑病血清中IL-23的表达水平与相应的PASI评分的值存在直线相关关系,且为正相关,说明血清中IL-23的表达水平可以反映疾病严重程度。 4寻常型银屑病患者血清中IL-27表达水平与正常对照组差异无统计学意义,说明IL-27的表达水平与寻常型银屑病的发病机制可能无明确关联,有待进一步研究。
