【摘要】:目的:应用系统评价的方法探讨低频重复经颅磁刺激(repetitive transcranial magnetic stimulation, rTMS)对脑卒中后失语的疗效与安全性,为失语症的辅助治疗方法提供证据。方法:计算机检索相关期刊论文(CNKI)、维普数据库(VIP)万方数据库、中国生物医学文献数据库(CBM)、PubMed、Science Direct、Web of Science,根据制定的纳入标准、排除标准收集低频rTMS治疗脑梗死后失语的随机对照试验(randomized control trials RCTs),采用Reviw Manager 5.3进行分析。结果:共纳入4篇RCTs,包括137例患者。Meta分析结果显示,失语总评分[SMD=0.71, 95%CI(0.28,1.13)]存在轻度异质性(I2=13%),但治疗后差异有统计学意义。复述亚测验组评分[SMD=0.54,95%CI (0.19,0.89)]、命名亚测验组[SMD=0.50,95%CI (0.05,0.95)]均无异质性(I2=0%),治疗后较治疗前有明显提高,差异有统计学意义。理解亚测验组评分[SMD=0.28,95%CI(-0.44,1.01)]有中度异质性(I2=54%),治疗前后差异无统计学意义。4项研究的所有受试者均无不良反应。结论:低频rTMS对脑卒中后失语患者语言功能的恢复具有明显效果,且无不良反应。
[Abstract]:Objective: To explore the evaluation method of the application system of low frequency repetitive transcranial magnetic stimulation (repetitive transcranial magnetic stimulation, rTMS) on the efficacy and safety of aphasia after stroke, to provide evidence for treatment of aphasia. Methods: computer retrieval China Journal Full-text Database (CNKI), VIP database Wanfang database (VIP), China the biomedical literature database (CBM), PubMed, Science Direct, Web of Science, according to the inclusion criteria, exclusion criteria to collect randomized controlled trials of low frequency rTMS in the treatment of cerebral infarction after aphasia (randomized control trials RCTs), using Reviw Manager 5.3 were analyzed. Results: 4 RCTs were included, including 137 cases of patients with the results of.Meta analysis show that the total score of [SMD=0.71 aphasia, 95%CI (0.28,1.13)] mild heterogeneity (I2=13%), but after treatment, the difference was statistically significant. Repeat the test sub group The score of [SMD=0.54,95%CI (0.19,0.89)], named sub test group [SMD=0.50,95%CI (0.05,0.95)] no heterogeneity (I2=0%) after treatment, significantly higher than before, the difference was statistically significant. Comprehension subtests score of group [SMD=0.28,95%CI (-0.44,1.01)] with moderate heterogeneity (I2=54%), all the subjects were statistically significant.4 study on the difference before and after treatment were no adverse reactions. Conclusion: low frequency rTMS on the recovery of language function in patients with aphasia after stroke has obvious effect, and no adverse reaction.
