有先兆偏头痛患者的静息态功能磁共振研究

【摘要】:目的:影像学研究已有发现无先兆偏头痛患者的脑结构及功能发生改变,本研究采用静息态功能磁共振成像技术来探讨有先兆偏头痛患者的自发神经元活动特征,分析偏头痛静息状态脑功能网络的变化,以便更好地认识有先兆偏头痛的发病机制。 方法:对7例发作间期有先兆偏头痛患者和7例年龄、性别及受教育程度相匹配的健康对照行静息态功能磁共振成像扫描,采用DPARSFA2.2软件分析原始数据,得出全脑低频振幅(amplitude of low frequencyfluctuation, ALFF),进行双样本t检验,并以ALFF差异的脑区为感兴趣区(region of interest,ROI)校正后行功能连接(Functional connectivity,FC)分析。 结果: 1两组局部脑区的ALFF值显示在图1及图2(P0.05).在两组的ALFF值均显著增高的脑区有前后扣带回皮层、楔前叶、下顶叶、内侧前额叶皮质、岛叶等。这些脑区均属于默认网络。 2与对照组相比,,病例组双侧额上回ALFF值显著高于对照组(P0.05);与对照组相比,病例组左侧眶额皮层ALFF值显著高于对照组(P0.05);病例组高于对照组的T值最高点在左侧额上回。 3与对照组相比,病例组左侧颞中回ALFF值显著低于对照组(P0.05);与对照组相比,病例组左侧颞下回ALFF值显著低于对照组(P0.05);与对照组相比,病例组左侧尾状核ALFF值显著低于对照组(P0.05);与对照组相比,病例组双侧丘脑ALFF值显著低于对照组(P0.05);与对照组相比,病例组右侧运动前区ALFF值显著低于对照组(P0.05);病例组低于对照组的T值最高点在左侧颞中回。 4与对照组相比,病例组小脑、岛叶、额叶、扣带回、缘上回、脑干与相关脑区的功能连接发生改变,具体为:与对照组相比,病例组左侧小脑、右侧脑岛、脑干与左侧额上回之间的功能连接减弱,病例组右侧眶额皮层、左侧额中回、双侧前扣带皮质、右侧缘上回与左侧额上回功能连接增强。 结论:有先兆偏头痛患者头痛发作间期疼痛相关脑区功能异常。这些发现有助于理解有先兆偏头痛的发病机制。
[Abstract]:Objective: imaging studies have found no changes in brain structure and function in patients with migraine aura. In this study, resting functional magnetic resonance imaging (fMRI) was used to investigate the characteristics of spontaneous neuronal activity in patients with migraine aura, and to analyze the changes of brain function network in patients with migraine. In order to better understand the pathogenesis of migraine aura. Methods: resting functional magnetic resonance imaging was performed in 7 patients with interictal migraine aura and 7 healthy controls with matched age, sex and education level. The DPARSFA2.2 software was used to analyze the original data and the low-frequency amplitude of low frequencyfluctuation was obtained. ALF FN, a double sample t test was performed, and the regions of interest were the regions of interest in which the ALFF difference was found in the brain regions. ROI) functional connectivity analysis was performed after correction. Results: 1the ALFF values of the local brain areas in both groups were shown in figs. 1 and 2 (P0.05). In both groups, there were anterior cingulate cortex, anterior cuneate lobe and inferior parietal lobe in brain regions with significantly increased ALFF values. Medial prefrontal cortex, insular lobe, and so on. These brain regions belong to the default network. 2Compared with the control group, the ALFF value of bilateral superior frontal gyrus in the case group was significantly higher than that in the control group (P 0.05). Compared with the control group, the ALFF value of the left orbital frontal cortex in the case group was significantly higher than that in the control group (P 0.05). The highest T value in the case group was higher than that in the control group in the left superior frontal gyrus. 3Compared with the control group, the ALFF value of the left middle temporal gyrus in the case group was significantly lower than that in the control group (P 0.05). Compared with the control group, the ALFF value of the left infratemporal gyrus in the case group was significantly lower than that in the control group (P 0.05). Compared with the control group, the ALFF value of the left caudate nucleus in the case group was significantly lower than that in the control group (P 0.05). Compared with the control group, the ALFF value of bilateral thalamus in the case group was significantly lower than that in the control group (P 0.05). Compared with the control group, the ALFF value of the right preexercise area in the case group was significantly lower than that in the control group (P 0.05). The highest T value in the case group was in the left middle temporal gyrus. 4Compared with the control group, the functional connections of cerebellum, insular lobe, frontal lobe, cingulate gyrus, superior marginal gyrus, brainstem and the related brain regions were changed in the case group, which were as follows: compared with the control group, the left cerebellum and the right cerebral island in the case group were changed. The functional connections between the brain stem and the left superior frontal gyrus were weakened. In the case group, the right orbital frontal cortex, the left middle frontal gyrus, the bilateral anterior cingulate cortex, the right superior marginal gyrus and the left superior frontal gyrus were enhanced. Conclusion: these findings are helpful to understand the pathogenesis of migraine with aura migraine.
