矿难所致PTSD患者静息状态下脑功能和神经网络的变化

【摘要】:实验一矿难所致PTSD患者静息状态下脑功能的变化 目的:探讨单一因素、高强度、长时间创伤暴露所致的PTSD患者静息状态下脑功能的变化。方法:采用自发性低频振幅(ALFF)的方法研究受试患者脑功能的变化,,研究对象为15名急性PTSD患者和15名健康对照。结果:与对照组相比,PTSD组右侧杏仁核、颞上回、双侧海马旁回ALFF升高;左侧苍白球和双侧丘脑ALFF减低。差异具有统计学显著性。结论:用ALFF的方法观察了PTSD患者大脑自发低频波动,发现患者边缘系统部分脑区ALFF升高;双侧丘脑、左侧苍白球ALFF减低。实验证明了PTSD患者调控情绪中枢活动异常,这些异常可能是PTSD发生发展的病理基础。 实验二矿难所致PTSD患者静息状态下脑神经网络的变化 目的:探讨矿难所致的PTSD患者脑神经网络的变化。方法:采用功能连接的方法研究受试者脑神经网络的变化,研究对象包括15名PTSD患者和15名健康对照。结果:PTSD组双侧岛叶、左侧扣带回中部、左侧中央旁小叶、右侧海马旁回、右侧颞上回、右侧楔叶、右侧小脑与后扣带回的功能连接较NC组显著升高。在P0.001(未校正),体素值15Voxel的显著性水平下未发现功能连接减低的结果。结论:用功能连接的方法对PTSD的致病机理进行了探讨,发现了PTSD患者脑内存在大量异常功能连接,这些异常可能是PTSD的神经病理机制。
[Abstract]:Changes of brain function in patients with PTSD induced by mine accident in resting state. Objective: to investigate the changes of brain function in patients with PTSD induced by single factor, high intensity and long time trauma exposure. Results: compared with the control group, ALFF in the right amygdala, superior temporal gyrus and bilateral pariprahippocampal gyrus increased in 15 patients with acute PTSD and 15 healthy controls. The difference between left globus pallidus and bilateral thalamus was statistically significant. Conclusion: the spontaneous low frequency fluctuation of brain in patients with PTSD was observed by ALFF. The decrease of ALFF in the left globus pallidus demonstrated that the abnormal regulation of emotional center activity in patients with PTSD may be the pathological basis of the pathogenesis and development of PTSD. Changes of brain neural network in PTSD patients induced by mine accident in experiment two under resting condition. Objective: to investigate the changes of brain neural network in PTSD patients caused by mine accident. Methods: the changes of brain neural networks in subjects were studied by functional connection, including 15 patients with PTSD and 15 healthy controls. Left cingulate gyrus middle, left central paracentral lobule, right parahippocampal gyrus, right superior temporal gyrus, right cuneate lobe, The functional connections between the right cerebellum and the posterior cingulate gyrus were significantly higher than those in the NC group. No decrease of functional connections was found at the level of P0.001 (uncorrected, voxel value 15 Voxel). Conclusion: the pathogenetic mechanism of PTSD was investigated by functional connectivity. A large number of abnormal functional junctions were found in the brain of PTSD patients, which may be the neuropathological mechanism of PTSD.
