非瓣膜性心房颤动患者血脂与缺血性卒中关系研究

【摘要】：目的:探讨血脂与非瓣膜性心房颤动(房颤)患者缺血性卒中的相关性。方法:非瓣膜性房颤患者2 470例,记录患者临床资料及入院后实验室检查结果。按有无缺血性卒中病史分为无缺血性卒中史组(1 910例)和有缺血性卒中史组(560例);根据CHA2DS2-VASc评分分为低中危组(2分,566例)和高危组(≥2分,1 904例),分别比较两组各项临床指标的差别,并进一步探讨非瓣膜性房颤患者血脂与缺血性卒中的关系。结果:无缺血性卒中史组与有缺血性卒中史组汉族比例、性别比、年龄、血红蛋白、红细胞压积、ALT、血尿酸、HDL-C、LDL-C差异均有统计学意义(P0.05)。低中危组和高危组汉族比例、性别比、年龄、白细胞计数、血红蛋白、红细胞压积、血小板计数、ALT、白蛋白、TG、LDL-C差异均有统计学意义(P0.05)。LDL-C为非瓣膜性房颤患者缺血性卒中史的独立危险因素(OR 2.089,95%CI1.860~2.347,P0.05),LDL-C为非瓣膜性房颤患者缺血性卒中风险的独立危险因素(OR 1.270,95%CI 1.079~1.494,P0.05)。结论:LDL-C与非瓣膜性房颤患者缺血性卒中相关,且为非瓣膜性房颤患者缺血性卒中的独立危险因素。
