年轻癫痫患者记忆功能与生活质量及其相关因素的研究

【摘要】：目的通过对癫痫患者及健康对照者临床记忆功能评估、对比，探索癫痫患者记忆损害特点，同时探讨多种临床因素（性别、年龄、起病年龄、文化程度、病程、发作频率、发作持续时间等）与记忆功能的相关性，探讨影响癫痫患者记忆功能的相关因素，探寻改善癫痫患者记忆功能或降低记忆损害的方法。材料与方法对102例癫痫患者及75例性别、年龄、文化程度相匹配的健康对照者同时进行临床记忆、生活质量、焦虑抑郁等量表测定，比较病例组和对照组这些指标的差别。同时详细搜集各个癫痫患者的临床资料，包括：性别、年龄、起病年龄、文化程度、病程、发作频率、发作持续时间、有无用药及用药数量，以及发作间期脑电图特点、头颅MRI或头颅CT等检查结果，并对这些临床因素与记忆功能进行相关分析。结果 1.癫痫患者临床记忆包括：记忆商、指向记忆、联想学习、图像自由回忆、无意义图形再认、人像特点回忆等均低于正常对照组（p0.01）。 2.癫痫患者生活质量评估显示生理、心理领域低于正常对照组（p0.05）。 3.单因素方差分析：性别、起病年龄、病程、发作频率、持续时间、用药情况与记忆商之间无明显相关（p0.05）。 4.方差分析显示文化程度较低的癫痫患者记忆功能更容易受损。 5.多元回归分析显示记忆商的影响因素是生活质量和文化程度。 6.影响癫痫患者生活质量的因素依次是焦虑、文化程度、病程、发病频率。结论 1.年轻癫痫患者记忆功能受损。 2.影响年轻癫痫患者临床记忆主要因素是生活质量和文化程度。 3.焦虑、文化程度、发病频率、病程是癫痫患者生活质量的影响因素 4.改善癫痫患者的生活质量，，尤其是焦虑，对降低记忆功能损害可能有一定价值。
[Abstract]:Objective to evaluate the clinical memory function of epileptic patients and healthy controls, and to explore the characteristics of memory impairment in epileptic patients, and to explore various clinical factors (sex, age, onset age, education level, course of disease). The relationship between seizure frequency, duration of seizure and memory function was discussed. The related factors affecting the memory function of epileptic patients were discussed, and the methods to improve the memory function or reduce the memory impairment in epileptic patients were explored. Materials and methods Clinical memory, quality of life, anxiety and depression were measured simultaneously in 102 epileptic patients and 75 healthy controls matched with sex, age and education. The difference of these indexes between the case group and the control group was compared. At the same time, the clinical data of each epileptic patient were collected in detail, including: sex, age, age of onset, education, course of disease, frequency of attack, duration of seizure, availability and quantity of medication, and characteristics of electroencephalogram during interictal period. Cranial MRI and CT were used to analyze the correlation between these clinical factors and memory function. Result 1. The clinical memory of epileptic patients including memory quotient, directed memory, associative learning, image free recall, meaningless pattern recognition, and characteristic recall of human image were lower than those of normal control group (p0.01). 2. The quality of life of epileptic patients was lower than that of normal control group (p 0.05). Univariate ANOVA: gender, onset age, course of disease, attack frequency, duration, drug use and memory quotient were not significantly correlated (p0.05). 4. Variance analysis showed that less educated epilepsy patients were more likely to suffer from memory impairment. 5. 5. Multiple regression analysis showed that the influencing factors of memory quotient were quality of life and education level. 6. 6%. The factors influencing the quality of life of epileptic patients were anxiety, education level, course of disease and frequency of incidence. Conclusion 1. Memory impairment in young epileptic patients. 2. The main factors affecting clinical memory of young epileptic patients are quality of life and education level. 3. 3. Anxiety, education level, incidence frequency and course of disease are the influencing factors of quality of life in epileptic patients. Improving the quality of life, especially anxiety, may be valuable in reducing memory impairment in epileptic patients.
