DWI在胶质瘤术后早期脑缺血诊断中的价值

【摘要】:目的研究弥散加权成像(DWI)对于脑胶质瘤术后早期缺血的诊断价值,探讨胶质瘤术后早期脑缺血的相关危险因素。 方法收集并分析8例脑胶质瘤患者手术前后MRI、DWI,以及患者个体因素、肿瘤相关因素、手术相关因素、治疗相关因素、治疗效果等方面的资料。 结果8例患者,1例患者术前肢体肌力下降,,手术及药物治疗后未见明显缓解,2例患者术后出现一侧肢体肌力下降,其中1例经药物治疗后恢复,另1例未完全恢复。影像学结果:1例患者术后DWI检查上出现片状高亮影,且病变累及内囊区,2例出现术区边缘少量高信号影,相应区域ADC值测量示明显较对侧降低,ADC图亦呈明显低信号;1例患者术区边缘出现少量混杂高信号,相应部位ADC图亦呈混杂稍低信号。 结论 1.DWI检查对于胶质瘤术后水肿及缺血等术后并发症有重要鉴别意义。 2.胶质瘤术后早期脑缺血DWI异常早于临床症状,为术后缺血性脑梗死的预防提供了治疗机会。
[Abstract]:Objective to study the diagnostic value of diffusion-weighted imaging (DWI) in early cerebral ischemia after glioma operation, and to explore the risk factors of early cerebral ischemia after glioma surgery. Methods the data of 8 patients with glioma before and after operation were collected and analyzed, including individual factors, tumor related factors, surgical factors, therapeutic effect and so on. Results the muscle strength of one patient was decreased before operation, but there was no obvious relief after operation and medication in 2 patients. One patient recovered after drug therapy, and the other one did not recover completely. The imaging results were as follows: 1 case showed flake highlight on DWI, and 2 cases with lesions involved in the internal capsule area showed a small amount of high signal intensity. The ADC value of the corresponding area was significantly lower than that of the contralateral decrease in ADC image. A small amount of mixed hyperintense signal appeared in the edge of the operation area in one patient, and the ADC map of the corresponding part also showed a slightly lower signal intensity. DWI has important significance in differentiating postoperative complications such as edema and ischemia after glioma. 2. The abnormal DWI of cerebral ischemia in early stage of glioma is earlier than that of clinical symptoms. It provides a therapeutic opportunity for the prevention of ischemic cerebral infarction after operation.
