延边地区朝汉族脑梗死患者同型半胱氨酸等生化指标及颈动脉斑块的研究

【摘要】:目的:通过探讨延边地区朝鲜族与汉族脑梗死患者血浆同型半胱氨酸水平(Homocysteine,HCY),颈动脉粥样硬化斑块及相关生化指标的关系,为脑卒中的二级预防提供参考。 方法:对2012年6月至2014年1月在延边大学附属医院神经内科住院治疗的142例脑梗死患者的临床资料进行了回顾性分析计,记录每位患者的性别、民族、年龄、合并高血压、糖尿病史情况、动脉硬化指数(Arteriosclerosis index,AI).超敏C反应蛋白(Hypersensitive C-reactive protein,hs-CRP).叶酸(Folic acid,FA).尿酸(Uric acid,UA).维生素B12(VitB,2)、血浆HCY和颈动脉超声结果,以60岁分组比较朝汉两个民族HCY.FA.AI、UA.VitB,2指标有无差异;根据颈动脉超声结果分为有斑块组及无斑块组,比较两组间HCY.FA.AI.UA. VitB12.hs-CRP指标有无差异;分析血浆HCY与FA、AI、UA、VitB12的相关性;按不同合并症分为单纯脑梗死组、脑梗死合并高血压组、脑梗死合并糖尿病组、脑梗死合并高血压糖尿病组,比较不同合并症组血浆HCY水平有无差异。 结果:脑梗死患者中朝鲜族较汉族的HCY值、AI值高,FA值则是汉族较朝鲜族高,不同年龄的HCY、AI、FA值不存在统计学差异;有斑块组的AI值高于无斑块组,差异有统计学意义(p0.01);有斑块组的hs-CRP值高于无斑块组,差异有统计学意义(p0.01),其余指标无统计学意义;HCY与FA存在负相关,相关系数为-0.53,p0.01,HCY与UA存在正相关性,相关系数为0.27,p0.01,与VitB12、AI不存在相关性;不同合并症组HCY值不完全相同,经过两两比较可知单纯合并高血压组、合并高血压糖尿病组的HCY值均高于对照组(p0.01);单纯合并糖尿病组与对照组HCY值差异不存在统计学差异(p0.05);单纯合并高血压组HCY值高于合并糖尿病组,差异具有统计学意义(p0.01);单纯合并高血压组与合并高血压糖尿病组HCY值比较差异无统计学差异(p0.05);合并高血压糖尿病组HCY值高于单纯合并糖尿病组(p0.01)。 结论:1.延边地区脑梗死患者中朝鲜族与汉族的HCY、AI、FA水平有差异;2.颈动脉斑块组较无颈动脉斑块组的AI、hs-CRP水平有差异;3.HCY与FA、UA有相关性:4.脑梗死患者中合并高血压组的较合并糖尿病组的HCY值升高。
[Abstract]:Objective: to investigate the relationship between plasma homocysteine (Homocysteine,HCY), carotid atherosclerotic plaque and related biochemical indexes in the patients with cerebral infarction in Yanbian area, so as to provide reference for the secondary prevention of stroke. Methods: the clinical data of 142 patients with cerebral infarction who were hospitalized in Department of Neurology, affiliated Hospital of Yanbian University from June 2012 to January 2014 were analyzed retrospectively. History of diabetes, arteriosclerosis index (Arteriosclerosis index,AI). Hypersensitive C-reactive protein (Hypersensitive C-reactive protein,hs-CRP). Folic acid (Folic acid,FA). Uric acid (Uric acid,UA). Vitamin B12 (VitB,2), plasma HCY and carotid artery ultrasound were compared in 60 years old with or without HCY.FA.AI,UA.VitB,2 between Korean and Han nationalities, and were divided into plaque group and non-plaque group according to the results of carotid ultrasound, and the HCY.FA.AI.UA. VitB12.hs-CRP indexes were compared between the two groups. To analyze the correlation between plasma HCY and FA,AI,UA,VitB12, according to different complications, the patients were divided into simple cerebral infarction group, cerebral infarction with hypertension group, cerebral infarction with diabetes group, cerebral infarction with hypertension diabetes group, cerebral infarction with hypertension diabetes group, cerebral infarction with hypertension diabetes mellitus group, cerebral infarction with hypertension diabetes group, cerebral infarction with hypertension diabetes group, cerebral infarction with hypertension diabetes group. To compare the plasma HCY levels in different complications. Results: in the patients with cerebral infarction, the HCY value, AI value and FA value of Korean nationality were higher than those of Han nationality, and the HCY,AI,FA value of different ages of Han nationality was higher than that of Korean nationality, the AI value of plaque group was higher than that of non-plaque group. The difference was statistically significant (p0.01), the hs-CRP value of plaque group was higher than that of non-plaque group, the difference was statistically significant (p0.01), the other indexes had no statistical significance, there was a negative correlation between HCY and FA, and the correlation coefficient was -0.53p0.01HCY and UA. The correlation coefficient was 0.27, p0.01, there was no correlation with VitB12,AI, the HCY value of the patients with different complications was not exactly the same, and the HCY values of the patients with hypertension and diabetes mellitus were higher than those of the control group (p0.01). There was no significant difference in HCY between the simple diabetes group and the control group (p0. 05), but the HCY value in the simple hypertension group was higher than that in the diabetes mellitus group, the difference was statistically significant (p0. 01). There was no significant difference in HCY between patients with hypertension and diabetes mellitus (p0. 05), and the HCY value in patients with hypertension and diabetes was higher than that in patients with hypertension and diabetes (p0. 01). Conclusion 1. There was significant difference in HCY,AI,FA level between Korean and Han in Yanbian area. The level of AI,hs-CRP in the carotid plaque group was significantly higher than that in the non-carotid plaque group, and there was a correlation between 3.HCY and FA,UA: 4. The HCY values in patients with cerebral infarction with hypertension were higher than those with diabetes.
