等速运动锻炼对老年脑卒中偏瘫患者下肢肌力学及步行能力的影响

【摘要】：目的探讨等速运动锻炼对老年脑卒中偏瘫患者下肢肌力学及步行能力的影响。方法 82例老年脑卒中偏瘫患者,按照随机数表法平均分为两组,其中对照组给予常规康复训练,观察组在常规康复训练的基础上加以等速运动锻炼,随访6个月后,采用改良巴氏指数(MBI)比较日常生活能力(ADL)、采用Fugl-Meyer运动功能评定FMA、采用Berg平衡量表评测平衡功能;比较两组6 min步行距离及10 m步行时间;采用改良Ashworyh量表比较两组肌张力变化。结果观察组MBI评分、FMA评分及BBS评分均高于对照组(均P0.05);观察组6 min步行距离大于对照组,10 m步行时间小于对照组(均P0.05);两组治疗前肌张力差别不大,观察组治疗后肌张力评分低于对照组(P0.05)。结论等速运动锻炼能够有效提高老年脑卒中偏瘫患者的下肢运动功能和平衡功能,对于降低患者的肌张力、提高患者的步行能力具有积极作用。
[Abstract]:Objective to investigate the effects of isokinetic exercise on lower limb myodynamics and walking ability in elderly stroke patients with hemiplegia. Methods 82 elderly stroke hemiplegic patients were divided into two groups according to the random number method. The control group was given routine rehabilitation training, the observation group was given isokinetic exercise on the basis of routine rehabilitation training, and the follow-up was 6 months later. The ability of daily living (ADL),) was compared with modified pasteurian index (MBI). Fugl-Meyer was used to evaluate the motor function of FMA, and Berg balance scale was used to evaluate the balance function. The walking distance of 6 min and walking time of 10 m were compared between the two groups, and the changes of muscle tension were compared with modified Ashworyh scale. Results the MBI score, FMA score and BBS score in the observation group were higher than those in the control group (P0.05), the walking distance of 6 min in the observation group was larger than that in the control group, and the walking time of 10 m in the observation group was smaller than that in the control group (P0.05). There was no significant difference in muscle tension between the two groups before treatment, and the score of muscle tension in the observation group was lower than that in the control group after treatment (P0.05). Conclusion Isokinetic exercise can effectively improve the lower limb motor function and balance function of the elderly patients with stroke hemiplegia. It can reduce the muscle tension of the patients and improve the walking ability of the patients.
