双侧上肢等速肌力训练结合针刺对脑卒中肩关节半脱位患者肩关节肌群表面肌电的影响和疗效

【摘要】：目的考察双侧上肢等速肌力训练结合针刺对脑卒中肩关节半脱位患者肩关节肌群表面肌电的影响及疗效。方法脑卒中肩关节半脱位患者120例按入院顺序随机分为对照组和观察组各60例。对照组给予针刺治疗,观察组给予双侧上肢等速肌力训练结合针刺治疗。结果与治疗前比较,角速度为60°/s时,两组治疗后三角肌和冈肌积分肌电值(i EMG)显著降低(P0.05);与对照组比较,观察组降低明显(P0.05);角速度为90°/s和120°/s时,两组三角肌和冈肌i EMG明显增高(P0.05);与对照组比较,观察组增高明显(P0.05)。治疗后,两组双侧肩峰至肱骨头间距明显减少(P0.05),两侧肩胛下角顶点间距明显增大(P0.05);与对照组比较,观察组更明显(P0.05)。与对照组比较,观察组治疗显效例数明显增多(P0.05)。结论双侧上肢等速肌力训练结合针刺对脑卒中肩关节半脱位有确切疗效,可能与影响肩关节肌群表面肌电有关。
[Abstract]:Objective to investigate the effect of isokinetic muscle strength training combined with acupuncture on the surface myography of shoulder muscles in patients with shoulder subluxation after stroke. Methods 120 patients with shoulder subluxation after stroke were randomly divided into control group (n = 60) and observation group (n = 60). The control group was treated with acupuncture, and the observation group was treated with bilateral isokinetic muscle strength training combined with acupuncture. Results compared with those before treatment, the (i EMG) of deltoid muscle and goniornus muscle in both groups decreased significantly after treatment when angular velocity was 60 掳/ s (P0.05), and that in observation group was significantly lower than that in control group (P0.05). When the angular velocity was 90 掳/ s and 120 掳/ s, the I EMG of deltoid and gonadal muscles increased significantly in both groups (P0.05), and that in the observation group was significantly higher than that in the control group (P0.05). After treatment, the distance between shoulder peak and humerus head in both groups decreased significantly (P0.05), and the distance between the two sides of the subscapular angle increased significantly (P0.05); compared with the control group, the observation group was more obvious (P0.05). Compared with the control group, the treatment in the observation group significantly increased the number of cases (P0.05). Conclusion bilateral upper limb isokinetic muscle strength training combined with acupuncture is effective in the treatment of shoulder subluxation after stroke, which may be related to the influence of surface myography on shoulder muscle group.
