25例成人丘脑恶性神经胶质瘤临床特点分析

【摘要】：目的:分析总结25例成人丘脑恶性神经胶质瘤临床特点。方法:回顾性分析2011—2016年首都医科大学附属北京世纪坛医院脑胶质瘤科收治的病理明确的25例成人丘脑胶质瘤患者,记录患者的症状持续时间、Karnofsky(KPS)评分、临床症状、治疗情况、无进展生存时间(PFS)和总生存时间(OS)。COX回归进行生存分析。结果:患者症状包括头晕头痛、肢体运动障碍、肢体感觉障碍、视力障碍、恶心呕吐、癫痫、记忆力下降和面瘫。11例患者行立体定向活检,14例患者行部分或近全手术切除,20例患者接受了同步放化疗和替莫唑胺化疗,25例患者的中位生存期为21.1个月(9.4~47.7个月),中位PFS 15.0个月(1.3~30.1个月)。多因素分析显示,症状持续时间(P=0.002)和KPS(P=0.003)是本组患者OS的独立预后因素。结论:症状持续时间超过2个月和KPS≥70分的患者预后较好。手术联合放化疗等综合治疗是成人丘脑胶质瘤主要的治疗方案。
[Abstract]:Objective: to analyze and summarize the clinical features of 25 adult thalamic malignant gliomas. Methods: a total of 25 adult patients with thalamic gliomas treated in the Department of glioma, Beijing Century altar Hospital, affiliated to Capital Medical University, from 2011 to 2016, were retrospectively analyzed. The symptom duration (, Karnofsky (KPS) score and clinical symptoms were recorded. Survival analysis was performed by (PFS) and total survival time (OS). COX) regression. Results: the symptoms included dizziness and headache, dyskinesia, sensory impairment, visual impairment, nausea and vomiting, epilepsy, memory loss and facial paralysis, 11 patients underwent stereotactic biopsy, 14 patients underwent partial or subtotal resection. The median survival time was 21.1 months (9.4 ~ 47.7 months) and 15.0 months (1.3 卤30.1 months) in 25 patients.
