VEGF复合藻酸钙提高脂肪移植成活率的实验研究

【摘要】:背景: 自体颗粒脂肪是一种良好的组织充填材料,取材方便,无组织排异性,采用注射方式进行游离移植,因而具有操作简易,创伤较小的优点。临床上,技术被广泛应用于面部及体表凹陷的充填,乳房形态的增大等整形美容领域。我们的前期研究证实:加速移植体的血管重建是提高颗粒脂肪移植存活率的关键,从而促血管生长因子成为了研究的重点。而如何选择组织相容性良好,适时降解,可缓释,无毒性的生物材料,构建生长因子复合载体,使生长因子可稳定释放,持续作用于靶组织和细胞是研究的关键和该领域的发展方向。 目的: 观察血管内皮生长因子(vascular endothelial growth factor, VEGF)与藻酸钙复合的缓释系统对自体脂肪移植成活率的影响。 方法: 从人体获取脂肪,经纯化,消化后按加入物不同分成4组,即实验组(脂肪+藻酸钙+20ng/ml VEGF)、藻酸钙对照组(脂肪+藻酸钙)、VEGF对照组(脂肪+20ng/ml VEGF)、空白对照组(单纯脂肪)。各组混合物分别注射于裸鼠背部皮下。在3、6和12周分别取材.行移植物质量、电镜超微结构、组织学染色和微血管CD34染色检测。 结果: 实验组(藻酸钙复合VEGF)的移植物重量和血管密度均高于对照组,具有统计学意义,其余各对照组间差别无统计学意义。 结论: 藻酸钙为载体构建的VEGF复合物能促进移植后脂肪组织血管的生成从而提高脂肪游离移植的成活率。
[Abstract]:Background: Autologous granular fat is a kind of good tissue filling material, which is easy to take, and can be used for free transplantation by injection, so it has the advantages of simple operation and less trauma. Clinically, the technique has been widely used in the plastic and cosmetic fields such as facial and body surface concave filling, enlargement of breast shape and so on. Our previous studies have confirmed that accelerated vascular remodeling is the key to improve the survival rate of granular fat transplantation, thus promoting vascular growth factor has become the focus of research. However, how to select biomaterials with good histocompatibility, timely degradation, slow-release and non-toxicity, to construct the compound carrier of growth factor, so that the growth factor can be released stably. Continuous action on target tissues and cells is the key of research and the development direction of this field. Objective: To observe the effect of vascular endothelial growth factor (VEGF) combined with calcium alginate on the survival rate of autogenous fat transplantation. Methods: Fat was obtained from human body, purified, digested and divided into 4 groups: experimental group (calcium alginate 20ng/ml VEGF), calcium alginate control group (adipose 20ng/ml VEGF), blank control group (simple adipose acid). The mixture was injected subcutaneously into the back of nude mice. The materials were collected at 3 ~ 6 and 12 weeks, respectively. The grafts mass, ultrastructure of electron microscope, histological staining and microvascular CD34 staining were detected. Results: The graft weight and vascular density of the experimental group (calcium alginate combined with VEGF) were significantly higher than that of the control group, but there was no significant difference between the other control groups. Conclusion: The VEGF complex constructed with calcium alginate as carrier can promote the angiogenesis of adipose tissue and improve the survival rate of free fat transplantation.
