基于微型压电与光谱生化分析系统的POCT新技术研究

【摘要】:微纳技术推动医学检验仪器向着微型化、自动化方向发展,促进了即时检验的出现。即时检验技术(POCT)具有简便快速、检测结果及时可靠等特点,,为疾病诊断提供了有效的辅助手段。POCT除了应用于临床诊断外,在现场急救、个人自检、社区医疗等领域也有广阔应用前景,正成为医学检验的重要发展方向。 首先本论文针对肾病、糖尿病诊断指标——尿微量蛋白的即时检验,采用微型压电生化分析系统,分别基于纳米金、普鲁士蓝的信号放大建立了相应的快速分析方法。其中,利用纳米金信号放大分别测定了尿液中微量的免疫球蛋白G(IgG)、白蛋白(Alb)、α1-微球蛋白(α1-MG)、β2-微球蛋白(β2-MG),其检测范围(μg/ml)分别为0-120、2-50、0.5-40和0.09-5;检出限(μg/ml)分别为0.84、1.54、0.98和0.085,检测时间均为25min;利用普鲁士蓝放大测定了尿液中微量IgG,检测范围为0-100μg/ml,检出限为0.38μg/ml,检测时间为25min。以上两种方法与免疫比浊法对照,应用于临床样品测定,t检验结果表明无显著性差异,均能满足医学检验要求。 其次本论文针对现场急救生化指标——Na~+、K~+、Cl~-、CO_2、CRE、Glu、Hb的即时检验,采用微型光谱生化分析系统,利用连续光谱测量实现了七项指标的同时、快速测定。其检测波长(nm)分别为620、620、480、405、600、505、540;检测范围(mmol/L)分别为0-200、0-8.0、0-120、0-75、0-1.15、0-20、0-3.0;检测时间为10min。以Na~+、Glu、Hb三项指标与全自动生化分析仪(奥林巴斯AU2700)对照,应用于临床样品测定,t检验结果表明无显著性差异,均能满足医学检验要求。 本论文采用微型化压电与光学生化分析系统,实现了肾病、糖尿病诊断指标及现场急救生化指标的快速检测,临床应用展示了由此建立新型即时检验技术的可行性,对于现场急救、个人自检、社区医疗等具有重要的应用价值。
[Abstract]:In addition to clinical diagnosis , POCT has broad application prospect in field first aid , personal self - test , community medical treatment and so on , which is becoming the important direction of medical examination .
In this paper , the rapid analysis method was established based on the signal amplification of nano - gold and pralumin blue . The detection range ( 渭g / ml ) was 0 - 120 , 2 - 50 , 0.5 -40 and 0.09 - 5 , respectively .
The detection limit ( 渭g / ml ) was 0.84 , 1.54 , 0.98 and 0.085 , respectively , and the detection time was 25 min .
The detection limit was 0 - 100 渭g / ml , the detection limit was 0 . 38 渭g / ml and the detection time was 25 min . The results of t - test showed no significant difference , which could meet the requirement of medical examination .
In this paper , a micro - spectrum biochemical analysis system was used to detect the biochemical indexes of Na ~ + , K ~ + , Cl ~ - , CO _ 2 , CRE , Glu and Hb in the field first - aid biochemistry index . The detection wavelength ( nm ) was 620 , 620 , 480 , 405 , 600 , 505 , 540 , respectively .

In this paper , the micro - type piezoelectric and optical biochemical analysis system is used to realize the rapid detection of renal disease , diabetes diagnosis index and on - site emergency biochemistry index . The clinical application shows the feasibility of establishing a new instant test technique , and has important application value for field first aid , personal self - test , community medical treatment and the like .
