不同方式重建股骨矩在老年股骨转子间骨折人工关节置换中的生物力学分布

【摘要】:背景:股骨矩在人工关节置换中具有重要作用,股骨矩的处理方法有骨水泥重建型和股骨矩骨折复位型,但关于那种处理方法更好的研究不多。目的:三维有限元分析股骨矩骨折复位型和骨水泥重建型重建股骨矩在老年股骨转子间骨折人工关节置换中的生物力学特点。方法:建立股骨矩骨折复位型(模型A)和股骨矩骨水泥重建型(模型B)老年股骨转子间骨折人工关节置换模型,观察2种模型骨界面上的应力分布、关节假体上应力分布、整体应力和两模型的位移大小。结果与结论:(1)模型A的骨界面上最大应变值和最大应力值均大于模型B;模型A的假体最大应变值和最大应力值均大于模型B;模型A骨界面上和假体上应力采集点、中应力采集点和下应力采集点的平均应力值均高于模型B的平均应力值(P0.05);(2)模型A的整体最大应力值和最小应力值均大于模型B;模型A的最大位移和最小位移均大于模型B;(3)结果表明,股骨矩骨折复位型老年股骨转子间骨折人工关节置换模型的最大应力和最大位移均大于股骨矩骨水泥重建型老年股骨转子间骨折人工关节置换模型,股骨矩骨水泥重建型老年股骨转子间骨折人工关节置换模型的应力分布更均匀,股骨矩骨水泥重建较股骨矩骨折复位效果好。
[Abstract]:Background: femoral moment plays an important role in artificial joint replacement. The treatment methods of femoral moment include bone cement reconstruction type and femoral moment fracture reduction type. Objective: to analyze the biomechanical characteristics of femoral moment reconstruction in elderly patients with intertrochanteric fracture by three dimensional finite element method. Methods: the artificial joint replacement models of femoral moment fracture (model A) and femoral moment cement reconstruction (model B) were established. The stress distribution at the bone interface and the stress distribution on the prosthesis of the two models were observed. Global stress and displacement of the two models. Results and conclusion the maximum strain value and maximum stress value at the bone interface of model A were larger than that of model B, the maximum strain value and maximum stress value of model A were larger than that of model B, and the stress collection points on the interface and prosthesis of model A were higher than that of model B. The average stress value of the middle stress acquisition point and the lower stress acquisition point is higher than that of model B (P < 0.05) the global maximum stress value and the minimum stress value of model A are higher than that of model B, and the maximum and minimum displacement of model A are both large. The results show that, The maximum stress and maximum displacement of the prosthetic joint replacement model of the senile intertrochanteric fractures were greater than those of the old intertrochanteric fracture replacement model. The stress distribution of the prosthetic joint replacement model of the aged intertrochanteric fracture was more uniform than that of the femoral moment cement reconstruction model, and the reduction effect of the femoral moment bone cement reconstruction was better than that of the femoral moment fracture.
