400例慢性非特异性下腰痛患者骨盆倾斜情况研究报告

【摘要】:目的:了解慢性非特异性下腰痛患者骨盆倾斜情况,从影像学表现探讨两者的相关关系。方法:收集自2014年7月至2014年10月在四川省骨科医院就诊并行影像学检查患者中筛选出符合条件患者的资料400例。运用四川省骨科医院影像系统软件对DR影像资料进行冠状位骨盆倾斜角、骨盆投射角、骶骨倾斜角、腰骶角测量,根据正常值范围对参数进行分组。通过SPSS统计分析观察:1.分组情况2.样本参数值与各参数标准均值的差异3.各参数之间及各参数与年龄、性别、病程关系。结果:1.冠状位骨盆倾斜角均值为1.66±1.23°;=0°组0例;0°且≤5°组388例,占97%;5°患者12例,占3%。2.SS均值34.12±8.05°;小SS组118例,占总体29.5%;正常SS组273例,占68.3%;大SS组9例,占2.3%。3.PI均值49.07±9.65°;小PI组69例,占17.3%;正常PI组308例,占77.0%;大PI组23例,占5.8%。4.LSA均值137.25±8.00°;小腰骶角组91例,占22.8%;正常腰骶角组300例,占75.0%;大腰骶角组,9例,占2.3%。5.骨盆在冠状面左倾172例,占43%,右倾228例占57%。左倾与右倾患者的年龄、性别、病程对数及影像学参数值无统计学差异。6.骨盆在矢状面后倾118例,占总体29.5%,前倾9例,占2.3%。7.SS、PI、LSA及(SS/PI)值较文献中正常值均值小且存在显著差异。8.不同性别患者LSA存在显著差异。9.SS与病程对数呈正相关;LSA与年龄、病程对数呈负相关;冠状位骨盆倾斜角与LSA呈正相关;LSA与SS呈负相关,SS与PI呈强正相关。结论:1.CNLBP患者骨盆冠状位倾斜普遍存在,而倾斜程度低。2.CNLBP患者骨盆矢状位倾斜以后倾为主,PI、SS、LSA较小者更易出现CNLBP症状。3.CNLBP患者中,女性LSA较男性LSA显著减小,腰骶关节失稳程度更大。4.CNLBP患者矢状面平衡对冠状面平衡有一定程度影响,而冠状面倾斜方向与矢状面倾斜方向无关。5.骨盆与腰椎在生物力学及相关功能上联系紧密,相关影像学参数密切相关。6.通过影像学检查明确CNLBP患者骨盆倾斜情况对诊断和治疗具有较强的指导意义。
[Abstract]:Objective: to investigate the pelvic obliquity in patients with chronic nonspecific low back pain and to explore the correlation between them by imaging findings. Methods: from July 2014 to October 2014, 400 eligible patients were selected from the patients who were admitted to Sichuan Orthopaedic Hospital for imaging examination. The imaging system software of Sichuan Orthopaedic Hospital was used to measure the coronal pelvic tilt angle, pelvis projection angle, sacral angle and lumbosacral angle. The parameters were grouped according to the normal range. Through SPSS statistical analysis observation: 1. Subgroup 2.
