不同分娩方式新生儿自动听性脑干反应听力初筛的最佳时间探讨

【摘要】:目的探讨应用自动听性脑干反应(automatic auditory brainstem response,AABR)对不同分娩方式新生儿进行听力初筛的最佳时间。方法应用Chirp声诱发的AABR(Chirp-AABR)对2016年8月1日至2016年10月31日出生的550例新生儿(顺产组300例,剖宫产组250例)进行听力初筛,顺产组各有100例分别在出生后24h、24~48h、48~72h三个时间段进行听力初筛,剖宫产组分别有50、100和100例在上述三个时间段进行听力初筛;未通过者42天采用畸变产物耳声发射和AABR进行复筛,复筛未通过者3个月内进行听力学诊断。结果顺产组新生儿出生后24~48h内初筛通过率(93.00%)显著高于24h者(83.00%)(χ~2=4.735,P=0.030.05),而与48~72h初筛者通过率(95.00%)比较差异无统计学意义(χ~2=0.355,P=0.550.05);剖宫产组新生儿出生后24~48h初筛通过率(83.00%)显著高于24h者(68.00%)(χ~2=4.371,P=0.040.05),而显著低于48~72h的通过率(94.00%)(χ~2=5.944,P=0.020.05)。结论顺产新生儿应用AABR听力初筛可在出生后24~48h进行,而剖宫产新生儿AABR听力初筛时间应选择出生后48h。
[Abstract]:Objective to explore the application of automatic auditory brainstem response in automatic auditory brainstem response. The best time of hearing screening for newborns with different delivery modes was obtained. Methods AABR Chirp-AABR induced by Chirp sound was used. From August 1st 2016 to October 31st 2016, there were 550 newborns (300 cases in the normal delivery group). Hearing screening was performed in 250 cases in cesarean section group, 100 cases in normal delivery group and 50 cases in caesarean section group at 24 hours after birth and 48 hours after birth, respectively. 100 and 100 cases were screened in the above three time periods. Distortion product otoacoustic emission (DPOAE) and AABR were used to screen the patients who failed for 42 days. Results the rate of primary screening pass within 24 to 48 hours after birth was significantly higher than that of 24 hours after birth in the normal delivery group (83.00%) than that in the 24 h group (83.00%) (P < 0.05). 蠂 2 + 4.735. There was no significant difference in the pass rate between the two groups (蠂 ~ (2 / 2) 0.35 ~ (5)) and that of the first sieve at 48 ~ 72 h (蠂 ~ (2 / 2) 0.355A ~ (0.550.05)), and there was no significant difference between the two groups (蠂 ~ (2 +)). The rate of primary screening in cesarean section group was significantly higher than that in 24 hours after birth (蠂 ~ 2, P = 4.371, P = 0.040.05), which was significantly higher than that in the group with 24 hours of cesarean section (蠂 ~ 2, P = 4.371a, P = 0.040.05). But the passing rate was significantly lower than that of 48 ~ 72h (蠂 ~ 2 + 5.944). Conclusion the primary screening of hearing with AABR can be carried out at 24 to 48 hours after birth, while the time of hearing screening for newborn with AABR in cesarean section should be chosen at 48 hours after birth.
