组织芯片技术研究眼眶淋巴瘤凋亡蛋白的表达及其临床意义

【摘要】:研究目的 运用组织芯片的相关技术分析眼眶淋巴瘤中凋亡相关基因Survivin、P53、 BCL-2的表达与其恶性程度和远期生存率的关系。 研究方法 首先收集46例眼眶非霍奇金淋巴瘤患者及17例眼眶淋巴组织增生患者组织标本蜡块;设46例眼眶淋巴瘤患者为实验组,17例眼眶淋巴组织增生患者为对照组,其中实验组再根据年龄、性别、病程、肿瘤大小及预后分组。同时搜集该63名患者的临床资料信息,并随访该46名眼眶淋巴瘤患者的预后情况; 其次制作淋巴瘤及淋巴组织增生的组织芯片,并对组织芯片进行免疫组化染色;镜下分析实验组及对照组Survivin、P53、BCL-2表达的阳性率; 最后应用SPSS17.0统计学软件对5、6两者的数据结果进行统计分析,以α=0.05为检验水准,P0.05认为有统计学意义。 研究结果 1、46例眼眶淋巴瘤和17例眼眶反应性淋巴增生组织芯片经HE染色后,病理诊断结果与原取材的大组织诊断完全一致。 2、三种凋亡相关蛋白,Bcl-2和P53在眼眶淋巴瘤和反应性淋巴组织增生的组织芯片中均有不同程度表达,Survivin在两种组织中均为阴性表达。Bcl-2蛋白主要表达于细胞膜,在两种病变的阳性表达率为84.8%和58.8%,其差异有统计学意义(P0.05);P53表达于细胞浆,在两种病变的阳性表达率为82.6%和52.9%,其差异有统计学意义(P0.05)。 3、46例眼眶淋巴瘤在预后不良组和预后良好组中,Bcl-2在两组间的表达率分别为96.4%和66.7%,其差异有统计学意义(P0.05);P53在两组间的表达率分别为92.9%和66.7%,其差异有统计学意义(P0.05)。 4、46例淋巴瘤:Bcl-2和P53这两种蛋白的阳性表达率在≥60岁和60岁年龄组间、男女性别组间、≥2.5cm和2.5cm肿瘤组间及≥1年和1年发病时间组间均无统计学意义。 5、Bcl-2和P53在眼眶淋巴瘤中的表达呈正相关(P0.05)。 结论 1、组织芯片技术高效、经济、快速、准确地研究了眼眶淋巴瘤的凋亡相关基因表达情况。 2、Bcl-2在眼眶淋巴瘤和反应性淋巴组织增生之间阳性率表达的差异有统计学意义,说明Bcl-2对判断眼眶淋巴细胞性疾病的良恶性有一定价值;同时Bcl-2在眼眶淋巴瘤的预后不良组和预后良好组之间阳性率表达差异有统计学意义,说明Bcl-2可以预测眼眶淋巴瘤预后。 3、P53在眼眶淋巴瘤和反应性淋巴组织增生之间阳性率表达的差异有统计学意义,说明P53对判断眼眶淋巴细胞性疾病的良恶性有一定价值;同时P53在眼眶淋巴瘤的预后不良组和预后良好组之间阳性率表达差异有统计学意义,说明P53可以预测眼眶淋巴瘤预后。 4、Survivin在眼眶淋巴瘤和反应性淋巴组织增生组中均呈阴性表达,提示Survivin与眼眶淋巴细胞性疾病的病变性质及预后无明确相关性。 5、Bcl-2和P53蛋白在眼眶淋巴瘤中的表达呈正相关性,提示联合检测这两种指标对预测眼眶淋巴瘤的预后有较大价值。
[Abstract]:Research purpose. The expression of apoptosis-related genes survivin p53 and BCL-2 in orbital lymphoma was analyzed by tissue microarray technique. The expression of survivin p53 and BCL-2 was correlated with the degree of malignancy and the long-term survival rate. Research method. At first, 46 cases of orbital non-Hodgkin 's lymphoma and 17 cases of orbital lymphoid tissue hyperplasia were collected, and 46 cases of orbital lymphoma were taken as experimental group and 17 cases of orbital lymphoid tissue hyperplasia as control group. The experimental group was grouped according to age, sex, course of disease, tumor size and prognosis. The clinical data of 63 patients were collected and the prognosis of 46 patients with orbital lymphoma was followed up. Then, the tissue microarray of lymphoma and lymphoid tissue proliferation was made, and the immunohistochemical staining of tissue microarray was carried out, and the positive rate of survivin P53 BCL-2 expression in experimental group and control group was analyzed under microscope. At last, the statistical analysis of the data of the two groups was carried out by using the SPSS17.0 statistical software, and the statistical significance was found to be statistically significant with 伪 0. 05 as the test level (P0.05). Research results. 1the pathological diagnosis of 46 cases of orbital lymphoma and 17 cases of orbital reactive lymphoid hyperplasia by HE staining was completely consistent with that of the original large tissue. 2, three kinds of apoptosis-related proteins Bcl-2 and p53 were expressed in orbital lymphoma and reactive lymphoproliferative tissue microarray to varying degrees. The expression of survivin was negative in both tissues. Bcl-2 protein was mainly expressed on cell membrane. The positive expression rates of p53 were 84.8% and 58.8 in the two kinds of lesions, and the difference was statistically significant. The positive expression rates of p53 in the cytoplasm were 82.6% and 52.9 in the two kinds of lesions, respectively. The difference was statistically significant (P 0.05). 3The expression rate of Bcl-2 was 96.4% and 66.7 in the poor prognosis group and the good prognosis group, respectively, and the difference was statistically significant. The expression rate of P0.05 and p53 in the two groups was 92.9% and 66.7, respectively. The difference was statistically significant. 4The positive expression rates of Bcl-2 and p53 proteins in 46 cases of lymphoma were not significantly different between the age group 鈮,
