正常幼儿内耳结构的MR测量

【摘要】:目的:建立1~5岁正常儿童内耳系统各主要结构的MRI测量正常值范围。 材料与方法:本论文选取了26例1~5岁健康儿童志愿者,共52耳行MRI水成像检查,将所有原始图像传送至工作站后进行三维重建,,多角度旋转并测量内耳各主要结构的径线值,并将各测量指标就性别、侧别间进行比较。 结果:(1)MRI内耳水成像技术可清晰显示正常内耳膜迷路各主要结构;(2)三条半规管高度性别间差异均有显著性意义(P0.05),分别为上半规管高度男性(5.86±0.33)mm与女性(5.65±0.11)mm;后半规管高度男性(5.68±0.33)mm与女性(5.38±0.40)mm;水平半规管高度男性(4.04±0.34)mm与女性(3.77±0.30)mm。其余测量指标性别间差异均无显著性意义(P0.05)。(3)所有测量指标侧别间差异均无显著性意义。(4)95%双侧范围分别为耳蜗高度(3.10~3.92)mm、底径(3.85~5.37)mm;前庭长径(4.94~5.96)mm、短径(3.02~4.24)mm;上半规管高度男性(5.02~6.70)mm与女性(4.87~6.43)mm、跨度(3.81~5.33)mm、管径(0.24~0.86)mm;后半规管高度男性(4.37~6.99)mm与女性(4.36~6.40)mm、跨度(1.83~3.05)mm、管径(0.26~1.12)mm;水平半规管高度男性(3.28~4.80)mm与女性(2.97~4.57)mm、跨度(3.26~4.60)mm、管径(0.26~0.92)mm。 结论:以幼儿内耳MR测量正常值范围的建立为基础,可以进一步完善先天性内耳畸形的诊断和分类方法,这对于幼儿耳聋的早期诊断和人工耳蜗植入术的成功实施都具有重要意义。
[Abstract]:Objective: to establish a range of MRI measurements of the main structures of the inner ear system in 5 years old children. Materials and methods: in this study, 52 ears of 26 healthy children aged 1 to 5 years were examined by MRI hydrography. All the original images were transferred to the workstation for 3D reconstruction, and the diameter of the main structures of the inner ear was measured and rotated at multiple angles. The measurement indexes were compared with each other in terms of sex and side. Results there were significant differences in the height of the three semicircular canals between men and women (5.86 卤0.33mm in upper semicircular canal and 5.65 卤0.11mm in female). The horizontal semicircular canal height was 4.04 卤0.34mm in males and 3.77 卤0.30mmmmmmmm. there was no significant difference in other measurements between males and females. The length of vestibular vestibular vestibule is 4.94 / 5.96mm, and the short diameter is 3.024.24mm; the upper semicircular canal height is 5.026.70mm for men and 4.876.43mm for women, with a span of 3.815.33mm and 0.240.86mm; the posterior semicircular canal height is 4.376.99mm for males and 4.366.40mm for females, and 1.833.05mm for diameter 0.261.12mm; the horizontal semicircular canal height is 3.284.80mm for males and 2.974.57mm for females, with a span of 0.2620.9mm. Conclusion: the diagnosis and classification of congenital inner ear malformation can be further improved on the basis of the establishment of normal range of Mr measurement in infants' inner ear. This is of great significance for the early diagnosis of deafness and the successful implementation of cochlear implantation.
