波前像差仪在激光原位角膜磨镶术前屈光度测量的意义

【摘要】:目的 波前像差理论在眼科学中的应用受到人们日益广泛的关注。波前像差仪不仅可以对人眼的高阶像差进行测量,而且也能应用于测量其低阶像差当中,应用波前像差技术可以对像差及眼科其他临床方面进行深入地分析。目前波前像差理论在分析人眼高阶像差方面较为广泛,而对于低阶像差的研究较少。 本文通过对不同验光方法进行对比研究,研究Zywave波前像差仪与常规验光对屈光度测量结果的准确性,探讨Zywave波前像差仪在准分子激光原位角膜磨镶术(laser in situ keratomileusis, LASIK)前测量屈光度的价值,为波前像差仪在测量低阶像差方面的应用提供理论依据。 方法 随机选取2011年6月至2011年8月间在我院准分子激光治疗中心接受准分子激光手术术前检查的近视者46人(84眼)。术前应用电脑自动验光、检影验光、主觉验光、综合验光、Zywave波前像差仪对其球镜度、柱镜度、等效球镜度测量并进行统计分析。 统计学分析:采用SPSS17.0统计学软件对所采集的数据进行统计学分析(配对t检验),均数用x±s)表示,检验水准a取0.05,P0.05作为有统计学差异的标准。 结果 1.低度近视组:电脑验光、检影验光分别与波前像差仪验光结果相比,球镜度、等效球镜度间差异有统计学意义,柱镜度间差异无统计学意义,综合验光、主觉验光分别与波前像差仪验光结果比较,三种屈光度间差异均无统计学意义。 2.中度近视组:电脑验光、综合验光、主觉验光、检影验光分别与波前像差仪验光结果比较,球镜度、等效球镜度间差异无统计学意义,柱镜度间差异有统计学意义。 3.高度近视组:电脑验光、综合验光、检影验光、主觉验光分别与波前像差仪验光结果比较,柱镜度间差异有统计学意义,球镜度和等效球镜度间差异无统计学意义。 结论 1.低度近视波前像差仪验光与综合验光结果有很好一致性。 2.中高度近视电脑验光柱镜结果可作为对波前像差仪验光柱镜结果的补充,以达到精调散光的目的。
[Abstract]:PurposeThe application of wavefront aberration theory in ophthalmology has attracted more and more attention.The wavefront aberration instrument can not only measure the high order aberrations of human eyes, but also can be used to measure the low order aberrations. The wavefront aberration technique can be used to analyze the aberrations and other clinical aspects of ophthalmology.At present, wavefront aberration theory is widely used in the analysis of high order aberrations of human eyes, but there is little research on low order aberrations.By comparing different optometry methods, the accuracy of Zywave wavefront aberration instrument and conventional optometry for diopter measurement is studied in this paper.To investigate the value of Zywave wavefront aberration measurement before laser in situ keratomileusism (LASIKI), and to provide a theoretical basis for the application of wavefront aberration in the measurement of low order aberration.MethodFrom June 2011 to August 2011, 46 patients (84 eyes) with myopia underwent excimer laser surgery in our hospital were randomly selected.Computer automatic optometry, shadow optometry and principal optometry were used before operation. The spherical, cylindrical and equivalent spherical specular degrees were measured and statistically analyzed by using a comprehensive refractive wavefront aberration instrument (Zywave wavefront aberration).Statistical analysis: the data collected were analyzed by SPSS17.0 software (paired t test, mean x 卤s).Result1.In the low degree myopia group: compared with the optometry results of wavefront aberration instrument, computer optometry and shadow optometry had statistical significance.There was no significant difference between the main refractive index and wavefront aberration instrument.2.In moderate myopia group, computer optometry, comprehensive optometry, principal optometry and shadow optometry were compared with those of wavefront aberration instrument respectively.3.In the group of high myopia, computer optometry, comprehensive optometry, optometry and principal optometry were compared with the optometry of wavefront aberration instrument respectively.Conclusion1.The results of low myopia wavefront aberration instrument are in good agreement with the results of comprehensive optometry.2.The results of computer optometry for medium and high myopia can be used to supplement the results of wavefront aberration instrument optometry in order to achieve the aim of fine adjusting astigmatism.
