奥曲肽玻璃体腔注射对视网膜新生血管的抑制作用及其机制探讨

【摘要】:目的:研究奥曲肽玻璃体腔注射对高氧诱导小鼠视网膜新生血管模型的抑制作用及其机制探讨。 方法:将健康的鼠龄7d天的昆明小鼠24只随机分为2组,正常组和高氧组,正常组8只,高氧组16只,正常组(A组)于正常空气环境中饲养5天,高氧组置于密闭的自制氧箱内饲养5天,高氧组又分为实验组(B组)和对照组(C组),每组各8只,鼠龄12天时,B组玻璃体腔内注射奥曲肽3μl,A组和C组玻璃体腔内注射等量生理盐水,给药后第7天,鼠龄19天时将其处死,摘除眼球,分别行视网膜铺片光镜下观察视网膜血管形态;HE染色计数突破视网膜内界膜的内皮细胞核平均数;免疫组化定量分析VEGF和IGF-1在视网膜内的表达。统计结果以均数士标准差表示,应用SPSS13.00软件进行统计分析,采用方差分析,组间两样本比较采用LSD-t检验,P<0.05认为差异有统计学意义。 结果:视网膜铺片A组视网膜血管均匀分布,管径较粗,分支良好,周边视网膜血管网结构清晰。B组视网膜血管分布不均匀,走行部分僵直,管径部分变细,视盘周围毛细血管部分闭塞,偶见无灌注区,周边视网膜分支小血管狭窄,无闭塞。C组视网膜血管分布不均匀,走行僵直,管径变细,视盘周围毛细血管闭塞,,见大片无灌注区,周边视网膜分支小血管狭窄,甚至闭塞,可见无灌注区。A、B、C组HE染色突破视网膜内界膜的血管内皮细胞核平均数呈增加趋势,分别为(0.27±0.23)、(1.77±0.38)及(3.57±0.71)个/HP,各组间差异显著(P<0.05),A、B、C组视网膜内VEGF的阳性表达有增强趋势,其平均灰度值测量依次为(160.82±5.90)、(127.14±4.40)、(104.22±3.18),各组间差异显著(P<0.05),A、B、C组视网膜内IGF-1的阳性表达亦有增强趋势,其平均灰度值测量依次为(145.23±5.58)、(126.65±5.13)、(105.14±4.49),同时视网膜内VEGF与IGF-1的表达,运用Spearman等级相关检验显示,二者有正相关性(P0.01)。 结论:奥曲肽能有效抑制小鼠视网膜新生血管的形成,同时降低VEGF和IGF-1在视网膜内的表达水平。
[Abstract]:Aim: to study the inhibitory effect of octreotide intravitreal injection on hyperoxia-induced retinal neovascularization in mice and its mechanism.Methods: Twenty-four Kunming mice aged 7 days were randomly divided into two groups: normal group and hyperoxia group, 8 normal mice, 16 hyperoxia group, and normal group A) were fed in normal air environment for 5 days.The hyperoxia group was kept in an airtight self-made oxygen box for 5 days. The hyperoxia group was divided into two groups: the experimental group (group B) and the control group (group C, 8 rats in each group).At 12 days of age, octreotide 3 渭 l of octreotide was injected into the vitreous cavity of rats in group B and group C was injected with the same amount of normal saline into the vitreous cavity. On the 7th day after administration, the rats were killed at 19 days of age and their eyeballs were removed.The average number of endothelial nuclei that broke through the retinal inner boundary membrane was counted by HE staining and the expression of VEGF and IGF-1 in the retina were quantitatively analyzed by immunohistochemistry.The statistical results were expressed as the standard deviation of mean value. The statistical analysis was carried out by using SPSS13.00 software. The analysis of variance was used. The comparison between the two samples was compared by LSD-t test (P < 0. 05) and the difference was statistically significant (P < 0. 05).Results: in group A, the retinal vessels were evenly distributed, the diameter of retinal vessels was coarse, the branches were good, the structure of peripheral retinal vascular network was clear and the distribution of retinal vessels in group B was not uniform.The peripheral capillaries were partially occluded, there was no perfusion area, the peripheral retinal branches were narrow, the retinal vessels in group C were not evenly distributed, the retinal vessels were stiffened, the diameter of the vessels became thinner, and the capillaries were occluded around the optic disc.A large area without perfusion was seen, and the peripheral retinal branches were narrow or even occluded. It was found that the number of endothelial nuclei in the non-perfused area.The positive expression of VEGF in retina of group A (0.27 卤0.23) was 1.77 卤0.38 (P < 0.05) and that of group C (3.57 卤0.71) was 3.57 卤0.71). The average gray value was 160.82 卤5.90 (127.14 卤4.404.40 卤104.22 卤3.18), and the positive expression of IGF-1 in retina was also increased in group C (P < 0.05).The average grayscale value was 145.23 卤5.58, 126.65 卤5.13, 105.14 卤4.49, and the expression of VEGF and IGF-1 in retina was positively correlated with Spearman grade correlation test (P0.01).Conclusion: octreotide can effectively inhibit the formation of retinal neovascularization and decrease the expression of VEGF and IGF-1 in the retina.
