高亲水性软性角膜接触镜在治疗白内障术后切口渗漏中的应用

【摘要】：目的:评估高亲水性软性角膜接触镜在白内障超声乳化手术术后切口渗漏的治疗效果及安全性。方法:回顾性病例分析我科2014年1月—2016年6月我院行白内障超声乳化吸出联合人工晶体植入术后出现切开渗漏的病例18例18眼,术后除常规用药外,给予佩戴高亲水性软性角膜接触镜。术眼观察视力、眼压、前房深度及角膜缘切口愈合情况。结果:所有患者中除1眼因既往有抗青光眼滤过性手术,同时给予加压包扎外,其余17眼在佩戴高亲水性软性角膜接触镜后第3天前房深度及眼压基本恢复正常;所有患者术后戴镜后第7天视力检查均有明显提高。戴镜前前房深度为1.1±0.3mm,戴镜后第3天为2.1±0.28 mm,差异有统计学意义;戴镜前眼压为9.0±2.2mmHg,戴镜后第3天为15±2.4mmHg,差异有统计学意义;所有患者佩戴高亲水性软性角膜接触镜后,均无特殊不适症状;未发现有角膜不良反应症状,均无继发感染迹象。所有患者21天后取出高亲水性软性角膜接触镜,角膜荧光染色,均无切口渗漏。结论:对于白内障超声乳化吸出联合人工晶体植入术后切口渗漏,及时佩戴高亲水性软性角膜接触镜是一种安全有效的治疗方法。
[Abstract]:Objective: to evaluate the efficacy and safety of high hydrophilic soft contact lens in the treatment of incision leakage after phacoemulsification. Methods: 18 cases (18 eyes) of cataract phacoemulsification combined with intraocular lens implantation in our hospital from January 2014 to June 2016 were analyzed retrospectively. Wear high hydrophilic soft contact lenses. Visual acuity, intraocular pressure, anterior chamber depth and corneal limbus wound healing were observed. Results: except for one eye with anti-glaucoma filtering surgery and compression bandaging, the anterior chamber depth and intraocular pressure in 17 eyes were basically normal on the 3rd day after wearing high hydrophilic soft contact lens. The visual acuity of all patients improved obviously on the 7th day after wearing mirror. The depth of anterior chamber was 1.1 卤0.3 mm and 2.1 卤0.28 mm on the third day after wearing mirror, and the intraocular pressure was 9.0 卤2.2 mm Hg and 15 卤2.4 mm Hg on the third day after wearing mirror, there was significant difference in all patients after wearing high hydrophilic soft contact lens. There were no special symptoms, no corneal adverse reactions and no signs of secondary infection. High hydrophilic soft contact lenses were removed from all patients 21 days later, and no incision leakage was found. Conclusion: for incision leakage after phacoemulsification combined with intraocular lens implantation, it is safe and effective to wear high hydrophilic soft contact lens in time.
