前庭康复训练对良性阵发性位置性眩晕复位后残余症状的疗效分析

【摘要】：目的:探讨良性阵发性位置性眩晕(BPPV)患者成功手法复位后行康复训练对残余症状及生活质量的影响。方法:选取成功手法复位后的BPPV残余患者98例,随机分为3组:A组32例为对照组,不予任何处置;B组33例,指导患者进行家庭自主康复训练;C组33例,指导患者行Brandt-Daroff习服训练。采用眩晕残障量表(DHI)和残余症状持续时间进行评估。结果:前庭康复训练1周后,B、C组DHI分值均下降,差异无统计学意义(P0.05),B、C组DHI分值分别与A组比较,均差异有统计学意义(P0.01);康复训练4周后,B、C组DHI分值较A组均下降,B组下降较C组明显,均差异有统计学意义(P0.01)。残余症状持续时间比较,B组与C组差异无统计学意义(P0.05),B、C组均低于A组(P0.05)。结论:BPPV患者成功复位后适量的康复练习有助于促进前庭中枢的代偿,改善残余症状,提高患者的生活质量,可作为耳石症患者复位后残余症状的辅助治疗措施。
[Abstract]:Objective: to investigate the effect of rehabilitation training on residual symptoms and quality of life (QOL) in patients with benign paroxysmal positional vertigo (BPPV) after successful manual reduction. Methods: 98 cases of residual BPPV patients after successful manual reduction were randomly divided into 3 groups: control group (n = 32), group B (n = 33), group C (n = 33) and group C (n = 33). The dizziness disability scale (DHI) and the duration of residual symptoms were evaluated. Results: after 1 week of vestibular rehabilitation training, the scores of DHI in group C decreased, and there was no significant difference in DHI score between group C and group A. After 4 weeks of rehabilitation training, the DHI score of group C was significantly lower than that of group A and group B was significantly lower than that of group C (P 0.01). There was no significant difference between group B and group C in the duration of residual symptoms. Group C was lower than group A (P 0.05). Conclusion proper rehabilitation exercises after successful reduction of the patients with BPPV can promote vestibular center compensation, improve residual symptoms and improve the quality of life of patients with otolith, and can be used as an auxiliary treatment measure for residual symptoms after reduction in patients with otolith.
