鼻咽癌DNA倍体性及CD133蛋白表达与放疗近期疗效的关系

【摘要】:目的: 检测鼻咽癌组织的DNA倍体性及CD133蛋白表达情况,并探讨二者与鼻咽癌放射敏感性及放射治疗近期疗效的关系。 方法: 分别采用流式细胞术和免疫组织化学染色法检测56例初治鼻咽癌患者治疗前肿瘤活检标本的DNA倍体性及CD133蛋白表达情况。另取10例鼻咽良性病变组织标本作为对照。所有患者均采用常规方案放疗,放疗剂量达40Gy时按CT所示肿瘤退缩程度评价放射敏感性,放疗结束时按RECIST实体瘤疗效评价标准评价近期疗效。结合临床资料对56例鼻咽癌组织的DNA倍体性及CD133蛋白表达情况进行分析,并探讨其与鼻咽癌放射敏感性及放疗近期疗效的关系。 结果: 1、DNA异倍体及CD133蛋白在鼻咽癌中的表达率高于鼻咽部良性病变组织,P0.05。 2、DNA异倍体的表达与鼻咽癌临床分期有关:Ⅲ-Ⅳ期组的表达率(46.88%)明显高于Ⅰ-Ⅱ期组(20.83%),P0.05;另外,DNA异倍体的表达也与肿瘤局部侵犯情况有关:T3-T4期组的表达率(54.55%)明显高于T1-T2期组(23.53%),P0.05;DNA异倍体的表达与患者年龄、性别、淋巴结转移无关,P0.05。CD133蛋白的表达与鼻咽癌淋巴结转移情况有关:N2-N3期组的阳性率(56.67%)显著高于N0-N1期组(26.92%),P0.05;CD133蛋白表达与患者的年龄、性别、肿瘤局部侵犯情况及临床分期无关,P0.05。 3、DNA异倍体组放射敏感高于DNA二倍体组,P0.05。CD133蛋白阳性组放射敏感性低于CD133蛋白阴性组,P0.05。 4、DNA异倍体组放疗近期疗效与DNA二倍体组无显著差异,P0.05。CD133蛋白阳性组放疗近期疗效与CD133阴性组相比较差,P0.05。 5、DNA异倍体与CD133蛋白表达呈正相关,P0.05。 结论: 1、DNA异倍体及CD133蛋白在鼻咽癌中的阳性表达率均显著高于鼻咽良性病变组织,提示二者可能与鼻咽癌的发生有关。 2、DNA异倍体与鼻咽癌放射敏感性有关,CD133蛋白表达与放射敏感性及放疗近期疗效均具有相关性,提示二者在预测放射敏感性,制定个体化治疗方案方面具有一定的参考价值。 3、DNA异倍体及CD133蛋白在鼻咽癌中的表达呈正相关,,提示二者可能协同参与了鼻咽癌的发生发展。
[Abstract]:Objective: To detect the expression of DNA ploidy and CD133 protein in nasopharyngeal carcinoma (NPC), and to explore the relationship between them and the radiosensitivity of nasopharyngeal carcinoma (NPC) and the short-term curative effect of radiotherapy. Methods: DNA ploidy and CD133 protein expression were detected by flow cytometry and immunohistochemical staining in 56 patients with nasopharyngeal carcinoma before treatment. Another 10 specimens of nasopharyngeal benign lesions were taken as control. All patients were treated with conventional radiotherapy regimen. The radiosensitivity was evaluated according to the degree of tumor withdrawal shown by CT when the dose of radiotherapy reached 40Gy, and the short-term efficacy was evaluated according to the criteria for evaluating the efficacy of RECIST solid tumor at the end of radiotherapy. The expression of DNA ploidy and CD133 protein in 56 cases of nasopharyngeal carcinoma (NPC) was analyzed with clinical data, and the relationship between DNA ploidy and the radiosensitivity of nasopharyngeal carcinoma (NPC) and the short-term curative effect of radiotherapy was discussed.
