OSAHS患者戴用自行调节式口腔矫治器下颌骨位置变化的研究

【摘要】：目的:为下颌前伸式口腔矫治器治疗阻塞性睡眠呼吸暂停低通气综合征OSAHS（obstructive sleep apnea hypopnea syndrome）的下颌定位提供参考方法:客观方面采用X线头影测量分别对32例患者戴用自行调节式口腔矫治器于治疗前、医师经验位及患者调节位时下颌水平及垂直测量项目的具体数据，，并进行统计学处理；主观方面结合Helkimo主诉症状指数分析；主客观相结合得出相关结论。结果:医师经验位和患者调节位与治疗前比较时，测量项目均有差异；医师经验位和患者调节位比较时，除S-Ar-Go和H-MP测量数据相差不大且前者P值为0.95后者P值为0.843均大于0.05,其他测量项目均有显著差异（p0.05）。两个位置时自行调节式口腔矫治器作用的主诉症状指数都有改善，但在患者调节位时的改善要较医师经验位时的改善更显著。结论：自行调节式口腔矫治器的医师经验位和患者调节位的下颌骨位置存在差异。患者的主诉症状指数因医师经验位和患者调节位的不同而存在差异，患者调节位优于医师经验位。自行调节式口腔矫治器可以达到个性化治疗目的。
[Abstract]:Objective: to provide a reference for mandibular localization of obstructive sleep apnea hypopnea syndrome (OSAHS(obstructive sleep apnea hypopnea syndrome) treated with mandibular protruding oral appliance. Methods: using X-ray cephalometry to measure the specific data of mandibular horizontal and vertical measurement in 32 patients who were wearing self-adjusting oral appliance before treatment, experience position of physician and patient's adjustment position, and were analyzed statistically. Subjective aspect combined with Helkimo main complaint symptom index analysis, subjective and objective combined with the relevant conclusions. Results: there were differences in the measurement items between the physician's experience position and the patient's adjustment position, and the comparison between the physician's experience position and the patient's adjustment position. Except for S-Ar-Go and H-MP, there was no significant difference in P value (P = 0. 95) and P value (P = 0.843) > 0. 05 (P = 0. 05), but there was significant difference in other measurement items (P < 0. 05). The main complaint symptom index of the self-regulated oral appliance was improved in both positions, but the improvement was more significant in the adjustment of the patient's position than that in the physician's experience position. Conclusion: there are differences in the position of the mandible between the physician's experience position and the patient's position. The main complaint symptom index of the patients is different because of the difference of the physician's experience position and the patient's adjustment position, and the patient's adjustment position is superior to the physician's experience position. Self-adjusting oral appliance can achieve the purpose of individualized treatment.
