聋儿人工耳蜗植入术后声场听阈的评估研究

【摘要】:目的:先天性极重度感音神经性耳聋的聋儿在接受人工耳蜗植入(cochlearimplantation,CI)术后,应用声场测试各纯音频率的听阈值,评估听觉能力。 方法:极重度感音神经性耳聋并接受CI的48例患儿,,男33例,女15例。大前庭水管综合征(large vestibular aqueduct syndrome,LVAS)10例。对48例患儿根据年龄分组,再将48例患儿根据内耳正常发育及内耳畸形之一的LVAS分组,应用声场技术对上述患儿就250Hz、500Hz、1000Hz、2000Hz、4000Hz各纯音频率测试听阈值及进行听觉评估。统计方法:使用SPSS16.0统计学软件对资料进行分析,计量资料采用均数±标准差(X±S)表示。采用两独立样本t检验,P<0.05有统计学意义。 结果:CI术后声场测试≤3岁组250Hz听阈值(29.38±9.24)dB;500Hz听阈值(28.33±7.02)dB;1000Hz听阈值(37.29±7.22)dB;2000Hz听阈值(49.38±5.38)dB较3-6岁组{(39.58±11.60)dB、(37.71±6.59)dB、(43.96±8.34)dB、(55.21±6.16)dB}阈值均较低(P0.05)。在4000Hz频率≤3岁组听阈值(59.58±10.31)dB与3-6岁组听阈值(62.08±9.43)dB间无统计学意义(P>0.05)。CI术后正常内耳发育组1000Hz听阈值(51.00±6.99)dB较LVAS组1000Hz听阈值(62.00±8.88)dB较低(P0.05)。CI术后正常内耳发育组250Hz听阈值(34.5±10.39)dB;500Hz听阈值(42.00±10.33)dB;2000Hz听阈值(59.50±8.64)dB;4000Hz听阈值(69.50±4.97)dB较LVAS组{(35.00±5.27)dB、(50.50±10.66)dB、(62.50±7.55)dB、(73.00±6.75)dB}阈值均无统计学意义(P>0.05)。 结论:应用声场测试得到的各纯音频率的听阈值评估CI术后聋儿的听觉康复效果,是可行的;对该类患儿的听觉及言语康复有较大的临床指导意义。
[Abstract]:Aim: to evaluate the hearing ability of deaf children with congenital very severe sensorineural deafness after cochlear implantation. Methods: there were 48 children with severe sensorineural hearing loss and CI, including 33 males and 15 females. Large vestibular aqueduct syndrome in 10 patients with large vestibular aqueduct syndrome. According to age, 48 children were divided into four groups according to normal development of inner ear and LVAS of one of inner ear malformations. Sound field technique was used to test the hearing threshold and auditory evaluation of 250Hz ~ 500Hz ~ 500Hz ~ 1000Hz ~ 1000Hz ~ 2000Hz pure tone frequency. Statistical methods: the data were analyzed by SPSS16.0 software, and the mean 卤standard deviation (X 卤S) was used to measure the data. T test with two independent samples had statistical significance (P < 0.05). Results the auditory threshold of 250Hz was 29.38 卤9.24dB1 500Hz and 28.33 卤7.02dB1000Hz in the group of 鈮
