鼓膜成形术与药物保守治疗单纯慢性化脓性中耳炎临床疗效观察

【摘要】：目的:观察鼓膜成形术与药物保守治疗单纯慢性化脓性中耳炎的近远期疗效。方法:将2011年1月至2013年1月收治的80例单纯慢性化脓性中耳炎患者作为研究对象,按完全双盲法分为对照组与观察组2组,每组各40例。对照组仅给予药物保守治疗,观察组则在药物治疗的基础上给予鼓膜成形术治疗,比较2组治疗前后听力恢复情况,统计2组治疗不良反应发生率,2组均随访24个月,统计患者近远期疗效及复发情况。结果:观察组治疗6月、12月、24月后气道骨差分别为(15.2±2.6)d B、(10.6±3.1)d B、(9.1±2.3)d B,与对照组相比差异有统计学意义(P0.05);随访6月、12月、24月,观察组听力改善率(93.2%、95.5%、97.7%)及鼓膜愈合率(93.2%、97.7%、97.7%)与对照组相比差异均有统计学意义(P0.05);观察组治疗不良反应发生率为4.5%,复发率为2.3%,对照组不良反应发生率及复发率分别为31.0%、16.7%,2组上述指标对比差异有统计学意义(P0.05);观察组随访6个月治疗有效率为93.2%,末次随访有效率为97.7%,对照组则分别为57.1%、78.6%,2组近远期疗效对比差异有统计学意义(P0.05)。结论:在单纯型慢性化脓性中耳炎患者的临床治疗中,采用保守药物处理联合鼓膜成形术方案,可促进鼓膜愈合,改善听力,降低不良反应发生率,优化近远期疗效,复发率低,值得推广。
[Abstract]:Objective: to observe the short-and long-term effect of tympanoplasty and drug conservative treatment on chronic suppurative otitis media. Methods: 80 cases of simple chronic suppurative otitis media from January 2011 to January 2013 were divided into control group and observation group with 40 cases in each group. The patients in the control group were treated with conservative drug therapy, while the observation group were treated with tympanoplasty on the basis of drug therapy. The hearing recovery of the two groups was compared before and after treatment. The incidence of adverse reactions in the two groups were all followed up for 24 months. The short-and long-term curative effect and recurrence of the patients were analyzed. Results: in the observation group, the difference of airway bone in 6 months, 12 months and 24 months after treatment was 15. 2 卤2. 6 卤2. 6 卤3. 1 卤3. 1 卤2. 3 dB, respectively, which was significantly different from that of the control group (P 0. 05), and followed up for 6, 12, 24 months. Conclusion: in the clinical treatment of simple chronic suppurative otitis media, conservative drug treatment combined with tympanoplasty can promote the healing of tympanic membrane, improve hearing, reduce the incidence of adverse reactions and optimize the short-term and long-term results. Recurrence rate is low, it is worth popularizing.
