人工耳蜗植入术中发现耳蜗骨化2例报告及文献综述

【摘要】:耳蜗骨化是内耳炎性反应的后遗症,而各种疾病引发的内耳炎症反应同样可导致感音神经性聋,当感音神经性聋为双侧重度或极重度时,就需要人工耳蜗植入手术。但是当耳蜗骨化时,尤其是蜗管完全骨化时,在手术中将电极顺利插入耳蜗会是一项艰难的任务,对手术医生很有挑战性。本文回顾性分析浙江大学医学院附属第二医院耳鼻咽喉科在2004年4月至2015年12月期间全部人工耳蜗植入手术病例,共518多例,其中术中发现2例耳蜗骨化。本文予以详细报道该2例耳蜗骨化病例,并检索复习国内外相关文献,总结了其常见病因及机制,探讨诊断方法以及手术技术,以期加强对耳蜗骨化的认识,提高术前诊断和术中处理技术。
[Abstract]:Cochlear ossification is the sequelae of inner ear inflammatory reaction, and the inflammatory reaction of inner ear caused by various diseases can also lead to sensorineural deafness. When sensorineural deafness is bilateral or extremely severe, cochlear implantation is required. However, when cochlea ossification, especially when cochlear canal ossification is complete, it is a difficult task to insert electrode into cochlea during operation, which is very challenging for the surgeon. From April 2004 to December 2015, more than 518 cases of cochlear implantation in the Department of Otorhinolaryngology, affiliated to the second affiliated Hospital of Zhejiang University Medical College, were retrospectively analyzed. Two cases of cochlear ossification were found during the operation. This paper reports in detail the two cases of cochlear ossification, reviews the relevant literatures at home and abroad, summarizes the common etiology and mechanism, probes into the diagnostic methods and surgical techniques, in order to enhance the understanding of cochlear ossification. To improve preoperative diagnosis and intraoperative management.
