阻塞性睡眠呼吸暂停低通气综合征患者糖化血红蛋白升高的相关因素

【摘要】：目的探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者糖化血红蛋白(Hb A1c)升高的相关因素。方法选取173例OSAHS患者,检测Hb A1c,并按标准分为Hb A1c升高组(53例),Hb A1c正常组(120例),应用SPSS21.0软件进行统计学分析。结果两组腰围身高比(WHtR)、呼吸暂停指数(AHI)、血氧饱和度(SaO_2)低于90%所占时间比(CT90%)、最低SaO_2(LSaO_2)、平均SaO_2(MSaO_2)、总呼吸暂停时间、最长呼吸暂停时间、氧减指数、腰围、颈围及OSAHS严重程度差异有统计学意义(均P0.05)。LSaO_2、MSaO_2与Hb A1c呈负相关(r=-0.222、-0.166,WHtR、CT90%、氧减指数、AHI、总呼吸暂停时间、腰围分别与Hb A1c呈正相关(r=0.276、0.216、0.251、0.209、0.166、0.272)。二元Logistic回归分析示:LSaO_2、WHtR进入模型(OR=0.950、2.052,P=0.001、0.007)。结论 OSAHS与Hb A1c升高密切相关,慢性间歇缺氧和中心性肥胖是OSAHS患者Hb A1c升高的独立危险因素。
[Abstract]:Objective to investigate the related factors of elevated HbA1c in patients with obstructive sleep apnea hypopnea syndrome (OSAHS). Methods 173 patients with OSAHS were selected to detect HbA1c, and were divided into 53 patients with elevated HbA1c and 120 patients with normal HbA1c. SPSS 21.0 software was used for statistical analysis. Results the ratio of waist circumference to height, apnea index (AHI), saturation of blood oxygen (SaO2) less than 90% in the two groups were less than CT90, and the lowest SaO2T / LSaO2C, the average SaO2S / MSaO2S, the total apnea time, the longest apnea time, the oxygen reduction index, waist circumference, There were significant differences in the neck circumference and the severity of OSAHS (all P0.05N. LSaO2MSaOS-2 and HbA1c were negatively correlated with HbA1c, oxygen reduction index (AHI), total apnea time and waist circumference were positively correlated with HB A1c. The binary logistic regression analysis showed that the weight of LSaOs 2WHtR entered the model OR0.950 / 2.052P0. 001 / 0.007. Conclusion OSAHS is closely associated with the increase of HbA1c. Chronic intermittent hypoxia and central obesity are independent risk factors for HbA1c elevation in OSAHS patients.
