慢性鼻-鼻窦炎临床表型及炎性特征的初步分析

【摘要】：目的:初步探讨慢性鼻-鼻窦炎的临床表型及炎性特征。方法:选取25例慢性鼻-鼻窦炎伴鼻息肉(CRSwNP)患者和23例慢性鼻-鼻窦炎不伴鼻息肉(CRSsNP)患者为研究对象,分析2种类型患者的临床特征(症状VAS评分、鼻内镜检查的Lanza-Kennedy评分、鼻窦CT的Lund-Mackay评分),比较病变组织中嗜酸粒细胞与中性粒细胞的分布情况,分析炎性标记物IL-5、IL-17、IFN-γ的表达情况。结果:CRSwNP患者的鼻塞VAS评分、鼻内镜评分及鼻窦CT评分均高于CRSsNP患者(P0.05)。苏木精-伊红染色显示,2种类型患者病变组织均为炎性细胞混合浸润。CRSwNP患者中10例以嗜酸粒细胞浸润为主,15例以嗜中性粒细胞为主;CRSsNP患者中18例以嗜酸粒细胞为主,5例以嗜中性粒细胞为主。CRSwNP患者IL-5、IL-17、IFN-γ的表达明显高于CRSsNP患者(P0.05)。结论:CRSwNP患者的临床表现及炎症程度比CRSsNP患者更严重。CRSwNP与CRSsNP病变黏膜组织均为炎性细胞混合浸润。
[Abstract]:Objective: to investigate the clinical phenotype and inflammatory characteristics of chronic rhinosinusitis. Methods: 25 patients with chronic rhinosinusitis with nasal polyps (CRSwNP) and 23 patients with chronic rhinosinusitis (CRSsNP) without nasal polyps were selected as subjects. The clinical features (symptom VAS score, Lanza-Kennedy score of nasal endoscopy) were analyzed. The distribution of eosinophils and neutrophils was compared and the expression of IL-5 and IL-17 IFN- 纬 was analyzed. Results the nasal congestion VAS score, nasal endoscopy score and sinus CT score were higher in the patients than those in the patients with CRSsNP (P0.05). Hematoxylin-eosin staining showed that the pathological tissues of two types of patients were inflammatory cell mixed infiltration. 10 cases of CRSwNP patients were mainly eosinophil infiltration 15 cases were mainly neutrophils and 18 cases were eosinophils. The expression of IL-5, IL-17 and IFN- 纬 in 5 patients with primary neutrophils was significantly higher than that in patients with CRSsNP (P0.05). Conclusion the clinical manifestations and inflammation degree of the patients with CRSsNP are more serious than those of the patients with CRSsNP. The mucosal tissues of the lesions of CRSwNP and CRSsNP are mixed infiltration of inflammatory cells.
