脉冲视野的短期波动及其影响因素的研究

【摘要】:目的:运用OCTOPUS 600全自动电脑视野计观察脉冲视野的短期波动情况以及白内障、年龄因素对脉冲视野测试结果的影响。方法:所有入选者分为两组:正常组和白内障组。正常组参与者是由青岛大学附属医院黄岛分院的职工、研究生及志愿者组成,共48例(48只眼),参与时间为2015年7月至2016年7月,所有入选的正常组参与者均于一周内的不同一天,分别进行一次脉冲视野测试,共三次,用时三天时间,分析这三次脉冲视野测试结果的平均敏感度(MS)、平均缺损(MD)及测试时间(t)有无差异,采用单因素方差分析进行统计学检验,P0.05有统计学意义。按照年龄因素,将正常组参与者分为三个年龄组:(30岁)组、(30-40岁)组、(40岁)组,观察年龄因素对第一次脉冲视野测试结果的平均敏感度(MS)、平均缺损(MD)及测试时间(t)的影响,采用线性回归分析进行统计学检验,P0.05有统计学意义。白内障组的入选者是由2015年7月至2016年7月期间于青岛大学附属医院黄岛分院眼科就诊的需行白内障超声乳化摘除联合人工晶体植入手术的患者组成,共26例(32只眼),所有入选的白内障患者分别于白内障术前和术后第12天进行一次脉冲视野测试,分析白内障手术前后两次脉冲视野测试结果的平均敏感度(MS)、平均缺损(MD)及测试时间(t)的变化,采用配对t检验进行统计学分析,P0.05有统计学意义。结果:在正常组中三次脉冲视野测试的MS值、MD值未见明显变化,P(MS)=0.706,P(MD)=0.733,两者均无统计学意义;而三次脉冲视野检查的测试时间是缩短的,P(测试时间)=0.008,有统计学意义;在第一次脉冲视野测试中,年龄因素与脉冲视野测试的MS值、MD值和测试时间之间均无明显关系,P(MS)=0.072,P(MD)=0.165,p(测试时间)=0.626,均无统计学意义。在白内障患者组中,白内障术后的MS值较术前显著提高,P0.01,有显著统计学意义;白内障术后的MD值较术前显著降低,P0.01,有显著统计学意义;白内障术后脉冲视野的测试时间较术前明显缩短,P0.01,有显著统计学意义。在所有入选者的脉冲视野测试结果中,MD值随着MS值的升高而降低。结论:正常组中脉冲视野的短期波动不明显,随着脉冲视野测试次数的增加,测试时间可能会缩短,白内障因素对脉冲视野的测试结果影响较显著,而年龄因素对脉冲视野的测试结果无明显影响。
[Abstract]:Aim: to observe the short term fluctuation of pulsed visual field and the effect of age and cataract on the results of pulsed visual field test with OCTOPUS 600 automatic computerized visual field meter. Methods: all the selected patients were divided into two groups: normal group and cataract group. The participants in the normal group were workers, graduate students and volunteers from Huangdao Branch, affiliated Hospital of Qingdao University. There were 48 patients (48 eyes) who participated in the study from July 2015 to July 2016. All participants in the normal group underwent a pulse field test on different days of the week, three times a total of three times, for three days. Analysis of the three pulse field test results of the average sensitivity (MS), average defect (MD) and test time (t) there were differences in the use of univariate ANOVA statistical test (P0.05) had statistical significance. According to the age factors, the normal subjects were divided into three age groups: (30), (30-40), (40) years old. The influence of age factors on the average sensitivity (MS),) of the first pulsed visual field test (PFT) and the test time (t) were observed. Linear regression analysis was used for statistical test (P0.05). The cataract group was selected by patients who needed phacoemulsification and intraocular lens implantation from July 2015 to July 2016 at the Huangdao Branch of Qingdao University Hospital. A total of 26 patients (32 eyes) with cataract underwent a pulse field test before and 12 days after cataract. Before and after cataract surgery, the mean sensitivity of the results of twice pulse field test (MS),) and the change of test time (t) were analyzed. The paired t test was used for statistical analysis (P0.05). Results: in the normal group, there was no significant change in the MS value and MD value of the three pulse visual field tests (P (MS) = 0.706 and P (MD) = 0.733), and there was no significant difference between the two groups, but the test time of the third pulse visual field test was shortened to 0.008, which was statistically significant. In the first pulse field test, there was no significant correlation between age factor and MS value, MD value and test time. There was no significant difference between P (MS) 0.072 P (MD) 0.165 p (test time) and 0.626 (test time). In the cataract group, the MS value after cataract operation was significantly higher than that before cataract (P 0.01), and the MD value after cataract surgery was significantly lower than that before cataract surgery (P 0.01). The test time of pulse visual field after cataract surgery was significantly shorter than that before operation (P 0.01). The MD value decreased with the increase of MS value. Conclusion: the short-term fluctuation of pulsed visual field is not obvious in normal group. With the increase of pulse field test times, the test time may be shortened. Cataract factors have a significant effect on the results of pulsed visual field test. However, age had no significant effect on the results of pulsed visual field.
