鼻内镜下两种手术方式治疗慢性泪囊炎的对比研究

【摘要】：目的：评价鼻内镜下泪囊鼻腔吻合联合泪囊支架植入术治疗慢性泪囊炎的远期临床效果。方法：将患者随机分成两组，研究组26例（33眼）患者采取鼻内镜下泪囊鼻腔吻合联合泪囊支架植入术，同时在支架周围填入美乐胶固定。对照组23例（28眼）患者采用单纯的鼻内镜下泪囊鼻腔吻合术，术中不应用泪囊支架，术毕用美乐胶把粘膜瓣固定。术后两组处理相同。随访12月。对比两组1月、6月、12月的治愈率和造瘘口的面积。结果：研究组与对照组术后1个月的治愈率分别为96.96%、92.86%，测定两组造瘘口面积分别为30.14±0.23mm~2、30.10±0.64mm~2，经统计学分析无显著性差异（P0.05）；两组随访6个月时治愈率分别为90.91%、71.43%；随访12个月时治愈率分别为84.85%、60.71%，经统计学分析存显著性差异（X2=3.889、4.560，P0.05）。两组术后6个月测量造瘘口面积分别为：23.95±3.07mm~2、12.57±2.26mm~2；术后12个月测量两组造瘘口面积分别为：20.12±2.34mm~2、10.15±1.85mm~2；经统计学分析有显著性差异(t=16.220、18.233,p0.05)。结论：鼻内窥镜下泪囊鼻腔吻合联合泪囊支架植入术治疗慢性泪囊炎有利于提高术后远期疗效，是一种创伤小，，安全有效的新治疗方法。
[Abstract]:Objective: to evaluate the long-term clinical effect of endoscopic dacryocystorhinostomy combined with dacryocyst stent implantation in the treatment of chronic dacryocystitis. Methods: the patients were randomly divided into two groups. 26 patients (33 eyes) in the study group were treated with endoscopic dacryocystorhinostomy combined with dacryocyst stent implantation. 23 patients (28 eyes) in the control group were treated with dacryocystorhinostomy under nasal endoscope without dacryocystorhinostomy. The two groups were treated the same after operation. Follow up for 12 months. The cure rate and the area of the fistula were compared between the two groups in January, June and December. Results: the cure rate of the study group and the control group was 96.96 卤92.86, respectively, and the area of the fistula was 30.14 卤0.23 mm / 2 respectively. There was no significant difference between the two groups by statistical analysis (P0.05), and the cure rate of the two groups was 90.9171.43 at 6 months follow-up. At 12 months follow-up, the cure rates were 84.85 and 60.71, respectively, and there was significant difference between them (X2P 3.889 4.560 P 0.05). In the 6 months after operation, the area of the fistula orifice in the two groups was: 23.95 卤3.07mm / 212.57 卤2.26mm / 2, respectively, and the area of the fistula orifice was 20.12 卤2.34mm / 210.15 卤1.85mm / 2 respectively at 12 months after operation. There was a significant difference between the two groups (t16.22018.233p0.05). Conclusion: endoscopic dacryocystorhinostomy combined with dacryocystorhinostomy is a safe and effective new method for the treatment of chronic dacryocystitis.
