阻塞性睡眠呼吸暂停低通气综合征嗜睡评价新方法研究—慢眼动分析

【摘要】:[目的]探索慢眼动潜伏期对OSAHS患者嗜睡评估的价值。[方法]连续入选2011年9月至2011年11月在北京协和医院睡眠呼吸疾患诊疗中心进行整晚多导睡眠监测,并确诊为重度OSAHS的患者20例。每位患者分别进行MSLT和MWT,经人工睡眠分期及慢眼动分析后,计算出慢眼动潜伏期及睡眠潜伏期并求出各自平均值,应用统计学方法进行一致性及相关性分析。同时每位患者还应用Epworth嗜睡量表进行主观嗜睡评估。[结果]不论MSLT还是MWT检查,SEM潜伏期均短于SL。MSLT平均SEM潜伏期与平均睡眠潜伏期相比(4.17±1.94min versus5.34±1.97min,p0.001),MWT平均SEM潜伏期与平均睡眠潜伏期相比(12.03±9.67minversus12.99±9.76min,p0.001)。MSLT及MWT的平均SEM潜伏期与平均睡眠潜伏期正相关,分别为(r=0.958,p0.001)和(r=0.997,p0.001);20名入组患者中18名患者Epworth嗜睡量表评分≥11分。MSLT中95%入组患者平均SEM潜伏时间≤8min;MWT中45%入组患者平均SEM潜伏时间≤8min。ESS评分与MSLT平均SEM潜伏期负相关,而在MWT中,两者未发现相关性。[结论]SEM潜伏期短于SL,两者正相关,通过SEM监测可以预测SL,从而进行嗜睡评价。
[Abstract]:[objective] to explore the value of slow eye movement latency in the evaluation of somnolence in patients with OSAHS. [methods] Twenty patients with severe OSAHS were selected from September 2011 to November 2011 to monitor polysomnography throughout the night at the diagnosis and treatment center of sleep respiratory diseases in Peking Union Hospital. After MSLT and MWT, were analyzed by artificial sleep stage and slow eye movement, the latent period of slow eye movement and sleep latency were calculated and their mean values were calculated. The consistency and correlation were analyzed by statistical method. At the same time, each patient was evaluated with Epworth somnolence scale. [results] in both MSLT and MWT, the latency of SEM was shorter than that of the average SEM latency of SL.MSLT and the average sleep latency (4.17 卤1.94min versus5.34 卤1.97 min / p 0.001).
