2014年天津市肠道病毒所致疱疹性咽峡炎临床流行病学分析

【摘要】：目的探讨天津市疱疹性咽峡炎的发生情况及其流行特征。方法采集了134例疱疹性咽峡炎患者中的133份咽拭子标本和54份粪便标本并进行肠道病毒(enterovirus,EV)检测,同时入户对所有患者进行流行病学调查。结果 134例疱疹性咽峡炎患者EV阳性率为64.93%,其中柯萨奇病毒A10(coxsackie virus A10,Cox A10)占34.48%、柯萨奇病毒A16(coxsackie virus A16,Cox A16)占29.89%。咽拭子标本的阳性率和粪便标本的阳性率一致性较高(Kappa值=0.709,P0.001)。临床症状中发热、咽痛、呕吐、惊厥的比例均高于同期手足口病患者(均有P0.05)。不同月疱疹性咽峡炎的主要流行病原不同(醊2=28.652,P0.001),且与同期手足口病流行的主要病原不同(均有P0.05),但总体上病原种类一致。结论作为肠道病毒传染源之一的疱疹性咽峡炎患者应与手足口病一起纳入监测管理。
[Abstract]:Objective to investigate the occurrence and epidemic characteristics of herpes isthmus in Tianjin. Methods 133 pharyngeal swabs and 54 feces were collected from 134 patients with herpes spondylitis and detected for enterovirus (enterovirus,EV). Results the positive rate of EV in 134 patients with herpes isthmus was 64.93. Coxsackie virus A10 (coxsackie virus A10 Cox A10 was 34.48%, Coxsackievirus A16 (coxsackie virus A16 Cox A16) accounted for 29.89%. The positive rate of pharynx swab was higher than that of fecal specimen (Kappa = 0.709). The rates of fever, pharynx, vomiting and convulsion in clinical symptoms were higher than those in patients with hand, foot and mouth disease (P0.05). The main epidemiological pathogens of herpes isthmus were different in different months (P 0.001), and were different from the main pathogens of hand, foot and mouth disease (P0.05), but the types of pathogens were the same as those of hand, foot and mouth disease (P0.05). Conclusion Herpes pharyngitis, one of the infectious sources of enterovirus, should be monitored and managed together with hand, foot and mouth disease.
