中鼻甲泡与鼻中隔偏曲及两者与慢性鼻—鼻窦炎关系

【摘要】:一、目的: 探讨球型中鼻甲泡的发生与鼻中隔偏曲是否相关,并分析两者与慢性鼻-鼻窦炎的关系。 二、方法: 1.972例(1944侧)患者的鼻窦冠状位CT片,为本研究各种统计数据的来源。 2.统计鼻中隔偏曲和无鼻中隔偏曲、高位偏曲和低位偏曲的球型鼻甲泡发生情况,并测量球型鼻甲泡横径,探讨球型鼻甲泡发生与鼻中隔偏曲的关系。 3.观察慢性鼻-鼻窦炎和球型鼻甲泡,并分析球型鼻甲泡与慢性鼻-鼻窦炎发生的关系。在CT片上测量球型鼻甲泡的横径数值,得出球型鼻甲泡的横径平均值,评估各球型鼻甲泡的大小。在同一CT片上,观察单侧和双侧有球型鼻甲泡者与慢性鼻-鼻窦炎的发生情况,并测量球型鼻甲泡横径,探讨其大小与慢性鼻-鼻窦炎的关系。 4.观察鼻中隔偏曲,把鼻中隔偏曲分为高位偏曲和低位偏曲,分析鼻中隔偏曲与慢性鼻-鼻窦炎发生的关系。 三、结果: 1.972例共1944侧的慢性鼻-鼻窦炎发生率为22.63%(440侧/1944侧)。球型鼻甲泡患者142例,发生率9.72%(189侧/1944侧),其中单侧球型鼻甲泡95例,双侧球型鼻甲泡47例。鼻中隔偏曲发生率18.72%(182例/972例),其中高位偏曲60例,低位偏曲122例。 2.在本组资料中,182例鼻中隔偏曲组球型鼻甲泡共31例,其中1例为双侧球型鼻甲泡,30例为单侧球型鼻甲泡,占8.79%(32侧/364侧),790例无鼻中隔偏曲组球型鼻甲泡共111例,其中48例为双侧球型鼻甲泡,63例为单侧球型鼻甲泡,占10.06%(159侧/1580侧),两组间球型鼻甲泡发生率无显著性差异(P0.05)。高位偏曲患者60例,存在单侧球型鼻甲泡20例,均位于鼻中隔凹侧,无双侧球型鼻甲泡,占16.67%(20侧/120侧),在低位偏曲患者122例中,出现单侧球型鼻甲泡10例,位于鼻中隔凹侧,双侧球型鼻甲泡1例,占4.92%(20侧/244侧),高位偏曲组与低位偏曲组球型鼻甲泡发生率有显著性差异(P0.05)。鼻中隔偏曲组和无鼻中隔偏曲组球型鼻甲泡横径均值分别为5.58±1.79mm和4.12±1.78mm,两者之间差异具有显著性(P0.01)。高位偏曲组和低位偏曲组球型鼻甲泡横径均值为5.92±1.87mm和4.80±1.35mm,两组间无显著性差异(P0.05)。 3.142例球型鼻甲泡患者中,95例单侧球型鼻甲泡者有鼻甲泡侧发生慢性鼻-鼻窦炎35侧,占36.84%(35侧/95侧),无鼻甲泡侧发生慢性鼻-鼻窦炎20侧,占21.05%(20侧/95侧),两组间慢性鼻-鼻窦炎发生率有显著性差异(P0.05)。47例双侧球型鼻甲泡组慢性鼻-鼻窦炎共47侧,占50.00%(47侧/94侧),830例无球型鼻甲泡组慢性鼻-鼻窦炎共338侧,占20.36%(338侧/1660侧),两组间慢性鼻-鼻窦炎发生率有显著性差异(P0.01)。单侧、双侧球型鼻甲泡不同鼻甲泡大小者慢性鼻-鼻窦炎发生率无显著性差异(P0.05)。 4.182例鼻中隔偏曲组慢性鼻-鼻窦炎有60例,占32.97%(60例/182例),无鼻中隔偏曲患者790例,有慢性鼻-鼻窦炎217例,占27.47%(217例/790例),两组间慢性鼻-鼻窦炎发生率无显著性差异(P0.05)。60例高位偏曲患组,发生慢性鼻-鼻窦炎27例,占45.00%(27例/60例),122例低位偏曲患中,发生慢性鼻-鼻窦炎33例,发生率27.05%(33例/122例),两组间慢性鼻-鼻窦炎发生率有显著性差异(P0.05)。无论高位偏曲还是低位偏曲,鼻中隔凸侧与凹侧慢性鼻-鼻窦炎发生率无显著性差异(P0.05)。 四、结论: 1.鼻中隔高位偏曲与球型鼻甲泡的发生有关,尤其是凹侧易出现球型鼻甲泡。 2.球型鼻甲泡与慢性鼻-鼻窦炎的发生有相关性,但慢性鼻-鼻窦炎的发生与球型鼻甲泡的大小无关。 3.鼻中隔高位偏曲与慢性鼻-鼻窦炎有相关性。
