个体化植入不同球差的IOL后不同全眼球差下视觉质量的对比研究

【摘要】:引言根据患者角膜球面像差选择不同球面像差的人工晶状体,满足患者对于视觉质量的要求,已经被许多专家认真研究和认可。本论文就是根据角膜球面像差为患者植入人工晶状体,以期达到术后最佳目标球差值,达到最佳视觉效果。目的植入不同球面像差的人工晶状体后,对比不同的全眼球面像差下人眼的视觉质量。方法收集2015年11月~2016年7月在郑州大学第一附属医院眼科本治疗小组接受白内障超声乳化+IOL植入术的年龄相关性白内障患者55例63眼,患者年龄范围为50~70岁,除了年龄相关性白内障以外,无其他眼部疾病,散光1.5D。分别依据患者术前角膜球面像差(spherical aberration,SA)植入不同SA的IOL,根据术后全眼SA值,将患者分为三组,A组术后的全眼SA为(-0.2±0.05)μm,B组为(0±0.03)μm,C组为(+0.1±0.03)μm。比较三组术后3月的裸眼视力(uncorrected visual acuity,UCVA)、最佳矫正视力(best-corrected visual acuity,BCVA)、全眼总高阶像差(high order aberration,HOA)、调制传递函数值(modulation transfer function,MTF)以及患者满意度调查。结果三组术后3月的UCVA,差异无统计学意义(P=0.153);三组术后3月的BCVA,差异无统计学意义(P=0.999)。术后3月的全眼HOA比较,三组之间差异无统计学意义(P=0.268)。在5周/度(c/d)、10周/度(c/d)、15周/度(c/d)、20周/度(c/d)、25周/度(c/d)、30周/度(c/d)的空间频率下,三组术后3月的MTF值,A组B组C组,差异有统计学意义(P0.05)。三组患者满意度调查A组B组C组,差异有统计学意义(F=19.700,P=0.000)。结论根据角膜球面像差个性化植入IOL,使得术后全眼球面像差为+0.1μm时,视觉质量较好,满意度较高。
[Abstract]:The introduction of the artificial lens with different spherical aberration according to the spherical aberration of the patient's cornea meets the requirements of the patient for the visual quality, and has been carefully studied and approved by many experts. In this paper, the artificial lens is implanted into the patient according to the spherical aberration of the cornea, so as to achieve the best target ball difference and achieve the best visual effect. Objective To compare the visual quality of human eyes with different spherical aberration after implantation of the artificial lens with different spherical aberration. Methods From November 2015 to July 2016,55 cases of age-related cataract with cataract phacoemulsification and IOL implantation were collected in the first affiliated hospital of Zhengzhou University in July,2016. The age range of the patients was 50-70 years. In addition to the age-related cataract, No other ocular diseases, astigmatism 1.5D. The patients with different SA were implanted according to the preoperative corneal spherical aberration (SA), and the patients were divided into three groups according to the postoperative full-eye SA value. The whole-eye SA in group A was (-0.2-0.05). m The group C was (+ 0.1-0.03). m u.m. The three groups were compared with the three-month open-hole vision (UCVA), best-corrected visual acuity (BCVA), total-eye total high-order aberration (HOA), modulation transfer function (MTF), and patient satisfaction survey. Results There was no significant difference in UCVA between the three groups (P = 0.153). The difference of BCVA and the difference in the three groups was not significant (P = 0.999). There was no significant difference between the three groups (P = 0.268).
