中国大陆鼻型NK/T淋巴瘤患者的预后因素及其重要性分析

目的:鼻型自然杀伤/T细胞(NK/T)淋巴瘤是世界卫生组织(WHO)肿瘤分类中的新类型。它在被发现时相对局限,但由于对化疗药物极易耐药且经常复发,所以预后不良。对该疾病特异性的治疗及影响预后的因素的研究尚无定论。本研究旨在探究EB病毒DNA,原发肿瘤体积(GTV-P)和Ki67作为潜在影响预后的因素在鼻型NK/T细胞淋巴瘤中所起的作用。方法:对51名鼻型NK/T细胞淋巴瘤患者进行前瞻性观察。对这51名患者及另外15名健康志愿者外周血EB病毒DNA进行实时定量PCR检测,GTV-P通过治疗计划系统软件通过电脑成像技术测得,而Ki67的数据来自每个病人组织病检报告。各病人其他的临床数据均来自各人的病例。结果:通过对EBNA-1片段测定发现29名患者中有20名是EBV-DNA阳性,而15名健康志愿者均为阴性。EBV-DNA和LDH水平峰值间存在正相关(p=0.002)。治疗后EBV-DNA水平普遍下降。单变量分析显示治疗前EBV-DNA水平与患者无进展生存率(PFS)相关(p=0.04)。具有高Ki67水平的患者PFS和无病生存率(DFS)均提高(p分别为0.03和0.04)。高GTV-P(...
