磁共振弹力成像与扩散加权成像诊断慢性乙型病毒性肝炎纤维化程度分级的对比

【摘要】:目的对比磁共振弹力成像(MRE)与扩散加权成像(DWI)诊断慢性乙型病毒性肝炎(CBH)纤维化程度分级的价值。方法回顾性收集CBH患者93例,均行肝穿刺活检,按照纤维化程度分级为S0-S4级;同期收集肝功能正常的健康查体者16例作为对照组。采用Spearman分析肝纤维化程度分级与弹性值、ADC值的相关性,采用ROC曲线分析肝弹性值、ADC值诊断纤维化程度分级的效能。结果患者组弹性值与纤维化程度分级呈显著正相关(r=0.962,P0.01),患者组ADC值与纤维化程度分级呈显著负相关(r=-0.823,P0.01);MRE/ADC检测肝脏≥S1、≥S2、≥S3、S4级的ROC曲线下面积(AUC)分别为0.963/0.868、0.995/0.947、0.998/0.948、0.996/0.889,它们之间差异均具有统计学意义(均P0.05)。结论 MRE和DWI诊断肝纤维化程度分级均有较高的价值,MRE诊断效能优于DWI。
[Abstract]:Objective to compare the value of magnetic resonance elastography (MRE) and diffusion-weighted imaging (DWI) in the diagnosis of fibrosis grade of chronic viral hepatitis B. methods 93 cases of CBH were collected retrospectively and all of them underwent liver biopsy and were classified as S0-S4 according to the degree of fibrosis. Sixteen healthy persons with normal liver function were collected as control group. The correlation between the degree of hepatic fibrosis and elastic value was analyzed by Spearman. The ROC curve was used to analyze the effectiveness of ROC value in the diagnosis of fibrosis grade. Results there was a significant positive correlation between the value of elasticity and the grade of fibrosis degree.
