益生菌对非酒精性脂肪性肝病患者细胞因子表达及肠道菌群的影响

【摘要】:目的探讨益生菌对非酒精性脂肪肝病(NAFLD)患者细胞因子及肠道菌群的影响及益生菌治疗NAFLD的疗效。方法选取NAFLD患者70例,采用随机数字表法分为常规治疗(A)组和联合治疗(B)组各35例。A组予多烯磷脂酰胆碱胶囊,B组予多烯磷脂酰胆碱胶囊联合口服枯草杆菌二联活菌肠溶胶囊。另纳入同期于该院体检者40例为对照(NC)组。比较3组间一般指标、肠道菌群、双歧杆菌/大肠杆菌(B/E)值、总胆固醇(TC)、甘油三酯(TG)、丙氨酸转氨酶(ALT)、门冬氨酸转氨酶(AST)、胰岛素抵抗(IR)指数(HOMA-IR)、白细胞介素(IL)-18、干扰素(IFN)-γ及IL-10水平。结果治疗前A、B组WC、BMI,TC、TG、HOMA-IR、ALT、AST、IL-18及IFN-γ水平均显著高于NC组,而肠道菌群数、B/E及IL-10水平明显低于NC组(P0.05);治疗后A、B组以上各指标水平均有改善,除B/E外,B组以上各指标改善均比A组更明显(P0.05)。结论 NAFLD患者血脂代谢絮乱、转氨酶及IR异常、肠道菌群及机体抗炎/促炎因子失调,益生菌可有效改善以上状态。
[Abstract]:Objective to investigate the effects of probiotics on cytokines and intestinal flora in patients with nonalcoholic fatty liver disease (NAF LDD) and the efficacy of probiotics in the treatment of NAFLD. The random digital table method was used to divide the two groups into routine treatment group (35 cases) and combined treatment group (35 cases). Group A was given polyenylphosphatidylcholine capsules (group B) and oral enteric capsules of Bacillus subtilis (Bacillus subtilis) combined with polyphosphatidylcholine capsule (group B). At the same time, 40 cases of physical examination in this hospital were used as control group. The general indexes were compared among the three groups. Intestinal flora, Bifidobacterium / Escherichia coli B / E, Results the levels of HOMA-IRN, IL-18, IFN- 纬, IFN- 纬 and TBMI-TGG in AB group were significantly higher than those in NC group before treatment, and the levels of Homa, IL-18, IFN- 纬 and IFN- 纬 in AB group were significantly higher than those in NC group before treatment, and there were no significant differences between the two groups before treatment, and the levels of IL-18 and IFN- 纬 in AB group were significantly higher than those in NC group before treatment, the levels of IL-18 and IFN- 纬 were significantly higher in AB group than those in NC group, and the levels of IL-18 and IFN- 纬 were significantly higher in group A than those in group C before treatment. However, the levels of B- / E and IL-10 in intestinal microflora were significantly lower than those in NC group (P 0.05), and the above indexes in group A and B were all improved after treatment, and the improvement of above indexes in group B was more obvious than that in group A (P 0.05). Conclusion the levels of lipid metabolism, transaminase and IR were abnormal in NAFLD patients. Intestinal flora and anti-inflammatory / pro-inflammatory factor disorder, probiotics can effectively improve the above state.
