慢性乙型肝炎肝脏超声半定量评分及其他指标与肝功能相关性研究

【摘要】：目的：探讨慢性乙型肝炎(Chronic hepatitis B, C H B)患者不同肝脏超声评分组之间肝大小指数、肝门静脉内径及流速、肝功能指标的差异,并分析超声评分、肝大小指数、门静脉内径及流速与肝功能指标的相关性。方法：对67例慢性乙型肝炎患者(男性患者43例,女性患者24例,年龄22-60岁,平均年龄48.3士9.6岁)的肝包膜、边缘角、实质、静脉壁及胆囊壁回声进行超声半定量评分,并测量肝大小指数、门静脉主干内径、门静脉主干血流速度。分析肝脏不同超声评分组之间的肝功能各项指标、肝脏大小指数、门静脉内径及血流速度的差异；研究超声评分、肝脏大小指数、门静脉内径及血流与肝功能指标之间的相关性。结果：①不同超声评分组的肝大小指数、门静脉内径及流速、肝功能指标比较差异有统计学意义(P0.05)。②血清总蛋白(Total Protein.TP)、清蛋白(Albumin.A l b)、前清蛋白(Prealbumin、P A)、胆碱酯酶(Cholinesterase.C H E)与超声评分、门静脉内径负相关；与肝大小指数、门静脉流速正相关(|r|均0.4,P0.05)。③血清碱性磷酸酶(Alkaline phosphatase.A l p)与超声评分、门静脉内径正相关；与肝大小指数、门静脉流速负相关(|r|均0.4,PO.05)。④超声评分与TP、Alb、P A、C H E、A l p的相关系数分别为r=-0.634、r=-0.761、r=-0.877、r=-0.763、r=0.722,相关系数较其他超声指标高。结论：肝脏超声评分、肝脏大小指数、肝门静脉内径及血流速度与肝功能指标变化有一定相关性,其中肝脏超声评分高低与肝功能关系更为密切。
[Abstract]:Objective: to study the difference of liver size index, portal vein diameter and velocity, liver function index between different liver ultrasound scoring groups in chronic hepatitis B patients with chronic hepatitis B hepatitis B (C-H B), and to analyze the ultrasonic score and liver size index. Correlation between portal vein diameter and flow velocity and liver function.
