MRI增强扫描中对比剂浓度及注射速率差异对胰腺图像质量影响的对照研究

【摘要】:目的:为提高病胰腺病变的检出率与确诊率,我们引入了对比剂,,以便增强正常组织与病变组织与的信号强度差。选择合适的对比剂,就会避免注射对比剂后患者产生一些不良反应。本研究对MRI对比剂的注射速率、浓度进行量化分析,对对比剂所产生的副反应进行详细观察,获得了比较合适的对比剂选择和注射方案。 材料与方法:选取胰腺MRI增强扫描的患者120例,排除其他疾病情况下的患者。选取腹主动脉、3处正常胰腺实质作为感兴趣区,分别测已选取的固定5处的信号强度,评价各组间腹主动脉,3处胰腺实质的信号强度,比较应用不同注射速率、不同浓度对比剂进行MRI增强扫描时图像质量的差异。 结果:浓度0.08mmol/kg,0.06mmol/kg的对比剂较浓度0.02mmol/kg的对比剂可以获得更佳的MRI图像,对比剂浓度的高低对患者产生的副反应并无明显差别。对比剂的注射速率2ml/s与注射速率3ml/s对胰腺MRI图像的成像质量无显著的影响,而注射速率3ml/s组较注射速率2ml/s组的患者引起的副反应发生率高。本次研究选取的120例患者行MRI检查前48h内检查肌酐(Cr)、尿素氮(BUN),检查后血浆Cr、BUN无升高。注射对比剂前后体征,包括:血压、脉搏、体温、无明显差异。本次研究选取的120例患者注射对比剂后有15例出现注射部位局部不同程度疼痛、肿胀,其中注射速率2ml/s的患者有4例,注射速率3ml/s的患者有11例。 结论:本实验对120例胰腺增强MRI检查患者的图像成像质量及对比剂产生的副反应进行分析,得出注射速率2ml/s,浓度0.08mmol/kg,0.06mmol/kg的对比剂在获得较好的MRI图像的同时减少了患者的副反应,成为本次研究得出的一组在胰腺成像中效价比高的对比剂使用参数。
[Abstract]:Objective: in order to improve the detection rate and diagnosis rate of pancreatic disease, we introduced contrast agent to enhance the signal intensity difference between normal tissue and pathological tissue. Choose the appropriate contrast agent, you will avoid some adverse reactions after injection of contrast agent. In this study, the injection rate and concentration of MRI contrast agent were analyzed quantitatively, the side effects of contrast agent were observed in detail, and the suitable contrast agent selection and injection scheme were obtained. Materials and methods: 120 patients with enhanced MRI were selected to exclude other diseases. Three normal pancreatic parenchyma of abdominal aorta were selected as regions of interest. The signal intensities of 5 fixed sites were measured to evaluate the signal intensity of pancreatic parenchyma at 3 sites of abdominal aorta, and different injection rates were compared. The difference in image quality of MRI enhanced scanning with different concentrations of contrast agent. Results: 0.08 mmol 路kg ~ (-1) 路kg ~ (-1) concentration of 0.06 mmol 路kg ~ (-1) contrast agent could obtain better 0.02mmol/kg images than that of 0.02mmol/kg, and there was no significant difference in the side effects of the concentration of contrast agent. Contrast medium injection rate (2ml/s) and injection rate (3ml/s) had no significant effect on the imaging quality of pancreatic MRI, but the incidence of side effects in the injection rate 3ml/s group was higher than that in the injection rate 2ml/s group. In this study, 120 patients were examined for creatinine bun and bun within 48 hours before MRI. Signs before and after injection, including: blood pressure, pulse, body temperature, there was no significant difference. There were 15 patients with local pain and swelling at the injection site after injection of contrast agent. Four patients with injection rate 2ml/s and 11 patients with injection rate 3ml/s were involved in this study. Conclusion: the imaging quality of 120 patients with enhanced MRI and the side effects of contrast media were analyzed. The results showed that the injection rate of 2 ml / s and the concentration of 0.08 mmol / kg 0.06 mmol / kg decreased the side effects of MRI images. As a result of this study, a group of high titer in pancreatic imaging contrast agent use parameters.
