不同程度脂肪肝患者声速值的变化

【摘要】:目的:通过测量不同程度脂肪肝患者肝组织的声速值,为脂肪肝的诊断提供量化依据,并初步探讨脂肪肝分度的临界值。方法:应用声速匹配技术测量140例受试者(轻、中、重度脂肪肝各31、38、33例,健康自愿者38例)肝组织区域速度指数(ZSI),计算声速值,分析不同程度脂肪肝患者肝组织的声速值变化,并构建差异有统计学意义的不同组别间的ROC曲线。结果:脂肪肝患者的声速值随着脂肪肝程度的加重而降低,差异有统计学意义(P0.05);正常对照组与轻度脂肪肝组、轻度脂肪肝组与中度脂肪肝组差异有统计学意义,中度脂肪肝组与重度脂肪肝组差异无统计学意义;正常对照组与轻度脂肪肝组、轻度脂肪肝组与中重度脂肪肝组临界声速值分别为1 545 m/s、1 505 m/s,ROC曲线下面积分别为0.96、0.93,约登指数分别为0.86、0.78,灵敏度分别为89.5%、93.5%,特异度分别为96.8%、84.5%。结论:肝组织声速值随着脂肪肝程度的加重而降低,声速匹配技术能够为脂肪肝的分度提供量化依据。
[Abstract]:Objective: to provide a quantitative basis for the diagnosis of fatty liver by measuring the sonic velocity of liver tissue in patients with fatty liver of different degrees, and to explore the critical value of fatty liver grading. Methods: the regional velocity index (ZSI) of liver tissue was measured in 140 patients with mild, moderate and severe fatty liver (31 ~ 833 mild, moderate and severe fatty liver, 38 healthy volunteers) by using sound velocity matching technique. The velocity of sound was calculated and the changes of sound velocity in liver tissue of patients with different degrees of fatty liver were analyzed. The ROC curves of different groups with statistical significance were constructed. Results: the sonic velocity of fatty liver patients decreased with the severity of fatty liver, the difference was statistically significant (P0.05), the difference between normal control group and mild fatty liver group, mild fatty liver group and moderate fatty liver group was statistically significant. There was no significant difference between moderate fatty liver group and severe fatty liver group. The critical sonic velocity values of mild fatty liver group and moderate severe fatty liver group were 1 545 m / s ~ (-1) ~ 1 505 m / s ~ (-1) ROC curve, respectively, respectively. The area of ROC curve was 0.966 卤0.93, the Yorden index was 0.86 ~ 0.78, the sensitivity was 89. 5% and the specificity was 96. 8% and 84. 5%, respectively. Conclusion: the sonic velocity of liver tissue decreases with the aggravation of fatty liver, and the technique of ultrasonic velocity matching can provide quantitative basis for the grading of fatty liver.
