乌司他丁联合血塞通注射液治疗老年急性胰腺炎的疗效及对血清IL-2、TNF-α及sICAM-1水平的影响

【摘要】：目的观察乌司他丁联合血塞通注射液治疗老年急性胰腺炎(AP)患者的疗效及对血清白细胞介素(IL)-2、肿瘤坏死因子(TNF)-α及可溶性细胞黏附因子(s ICAM)-1水平的影响。方法 80例老年AP患者随机分成对照组和试验组各40例。对照组给予乌司他丁治疗,试验组给予乌司他丁联合血塞通注射液治疗,两组接收治疗2 w。评价并比较两组临床疗效。检测并比较两组患者治疗前后血淀粉酶(BAMY)、尿淀粉酶(UAMY)水平及血清IL-2、TNF-α及sICAM-1水平。记录并比较两组患者治疗过程中出现的过敏、皮疹、白细胞下降等不良情况发生率。结果试验组总有效率明显高于对照组(χ~2=4.114,P=0.043)。治疗前,两组BAMY、UAMY水平及血清IL-2、TNF-α、s ICAM-1水平比较差异无统计学意义(P0.05);治疗后,两组均明显低于治疗前,且试验组均明显低于对照组(P0.05)。两组不良反应发生率比较差异无统计学意义(χ~2=0.213,P=0.644)。结论乌司他丁联合血塞通注射液治疗老年AP的临床疗效显著,能够明显降低BAMY、UAMY水平及血清IL-2、TNF-α、s ICAM-1水平,且安全性较好。
[Abstract]:Objective to observe the effect of ulinastatin combined with Xuesaitong injection on the serum levels of interleukin (IL)-2, tumor necrosis factor (TNF)-伪 and soluble cell adhesion factor (s ICAM)-1 in elderly patients with acute pancreatitis (AP). Methods 80 elderly patients with AP were randomly divided into control group and experimental group, 40 cases in each group. The control group was treated with ulinastatin, the experimental group was treated with ulinastatin combined with Xuesaitong injection, and the two groups were treated for 2 weeks. To evaluate and compare the clinical efficacy of the two groups. The levels of serum amylase (BAMY), urine amylase (UAMY) and serum IL-2 TNF- 伪 and sICAM-1 were measured and compared before and after treatment. The incidence of allergy, rash and leukopenia were recorded and compared between the two groups. Results the total effective rate in the trial group was significantly higher than that in the control group (蠂 2 + 4.114 P < 0.043). Before treatment, there was no significant difference in the levels of BAMYUAMY and serum IL-2TNF- 伪 ICAM-1 between the two groups (P0.05); after treatment, the two groups were significantly lower than before treatment, and the experimental group were significantly lower than the control group (P0.05). There was no significant difference in the incidence of adverse reactions between the two groups (蠂 ~ (2 +) 0.213 P ~ (0.644). Conclusion Ulinastatin combined with Xuesaitong injection is effective in the treatment of senile AP. It can significantly reduce the level of BAMYN UAMY and serum IL-2TNF- 伪 ICAM-1, and is safe.
