部分消化道疾病患者血清过敏原的临床研究

【摘要】:目的:分析慢性胃炎、肠易激综合征、肠息肉三种消化道疾病患者血清过敏原特异性IgE抗体的检测结果,探讨过敏因素与上述三种消化道疾病有无关联。 方法:选取2012年5月至2014年4月于大连医科大学附属第一医院消化内科住院患者150例。150例患者经临床确诊慢性胃炎55例,肠易激综合征35例,肠息肉60例。其中慢性胃炎诊断标准依据胃镜诊断及病理诊断,肠易激综合征诊断依据罗马Ⅲ IBS诊断标准,肠息肉的诊断依据肠镜检查和肠黏膜活检病理诊断。对照组为同一时期于我院体检的健康人150例。采用欧蒙印迹法,对消化道疾病患者及健康人进行血清20种常见过敏原特异性IgE抗体检测,分析过敏因素与上述三种消化道疾病有无关联,应用SPSS16.0软件统计分析。 结果: 1、三种消化道疾病共150例患者与对照组150例健康人共300例血清过敏原特异性IgE抗体检测结果提示:树组合、普通豚草、艾蒿、尘螨组合、屋尘、猫毛、狗上皮、蟑螂、霉菌组合、鸡蛋白、花生、黄豆、牛肉、海鱼组合、淡水鱼组合、虾及蟹的特异性IgE抗体检测结果为阳性;律草、羊肉、牛奶的特异性IgE抗体检测结果为阴性。 2、三种消化道疾病共150例患者与对照组150例健康人血清过敏原特异性IgE抗体检测结果依据浓度分类构成比比较,对照组150例健康人血清过敏原特异性IgE抗体检测结果,阴性占87.33%,特异性IgE抗体浓度达到1-2级占12.67%,特异性IgE抗体浓度≥3级占0%。患者组150例患者血清过敏原特异性IgE抗体检测结果,,阴性占42.67%,特异性IgE抗体浓度达到1-2级占34.67%,特异性IgE抗体浓度达到3级占10.67%,特异性IgE抗体浓度≥4级占12.00%。 3、三种消化道疾病共150例患者与对照组150例健康人血清过敏原特异性IgE抗体阳性率比较:150例患者血清过敏原特异性IgE抗体阳性率(57.33%)明显高于对照组150例健康人(13.33%),两者比较差异有统计学意义(P<0.05)。血清过敏原特异性IgE抗体阳性率最高的为肠易激综合征患者,其后依次为肠息肉患者及慢性胃炎患者,以上三种消化道疾病患者血清过敏原特异性IgE抗体阳性率与对照组比较差异均有统计学意义(P<0.05)。 4、三种消化道疾病共150例患者与对照组150例健康人血清吸入性过敏原特异性IgE抗体阳性率比较:患者组尘螨组合、狗上皮、蟑螂及树组合的血清特异性IgE抗体阳性率明显高于对照组,两者比较差异有统计学意义(P<0.05),患者组中特异性IgE抗体阳性率最高的吸入性过敏原为尘螨组合。 5、三种消化道疾病共150例患者与对照组150例健康人血清食入性过敏原特异性IgE抗体阳性率比较:患者组蟹、海鱼组合、淡水鱼组合、鸡蛋白、虾及黄豆的血清特异性IgE抗体阳性率明显高于对照组,两者比较差异有统计学意义(P<0.05),患者组中特异性IgE抗体阳性率最高的食入性过敏原为蟹。 6、三种消化道疾病共150例患者不同浓度水平血清吸入性过敏原特异性IgE抗体阳性率显示:在1级-2级水平,阳性率最高的过敏原为尘螨组合,在3级水平,阳性率最高的过敏原为尘螨组合,在≥4级水平,阳性率最高的过敏原为尘螨组合。 7、三种消化道疾病共150例患者不同浓度水平血清食入性过敏原特异性IgE抗体阳性率显示:在1级-2级水平,阳性率最高的过敏原为蟹,在3级水平,阳性率最高的过敏原为海鱼组合,在≥4级水平,阳性率最高的过敏原为蟹。 8、三种消化道疾病患者血清过敏原特异性IgE抗体检测结果阳性率两两比较:肠易激综合征患者血清过敏原特异性IgE抗体阳性率高于慢性胃炎患者,两者比较差异有统计学意义(P<0.05)。 9、不同性别患者血清过敏原特异性IgE抗体检测结果的阳性率比较:男性患者血清过敏原特异性IgE抗体阳性率为56.72%,女性患者血清过敏原特异性IgE抗体阳性率为57.83%,两者比较差异无统计学意义(P>0.05)。 结论:慢性胃炎、肠易激综合征、肠息肉三种消化道疾病患者均具有较高的血清过敏原特异性IgE抗体检测阳性率,其中血清过敏原特异性IgE抗体检测阳性率最高的吸入性过敏原为尘螨组合,血清过敏原特异性IgE抗体检测阳性率最高的食入性过敏原为蟹,过敏因素与上述三种消化道疾病有一定关联,该研究结果为今后进一步干预治疗提供一定的依据。
