中老年慢性胃病幽门螺杆菌感染与胃黏膜病理改变的相关性

【摘要】：目的探讨中老年慢性胃病患者幽门螺杆菌(Hp)感染与不同胃病、胃黏膜病理改变之间的关系。方法对631例50岁以上的慢性胃病患者行胃镜、病理活检和~(13)C-尿素呼气试验(~(13)C-UBT),观察不同年龄组、不同胃病组Hp感染情况,观察胃黏膜不同病理组织学改变情况。结果中老年慢性胃病患者HP感染率为61.17%。50~59岁、60~69岁、70~79岁、≥80岁组Hp感染率分别为64.26%、64.44%、58.42%、48.28%;慢性浅表性胃炎、慢性萎缩性胃炎、胃溃疡、十二指肠溃疡和胃癌(GC)Hp感染率分别为41.38%、60.08%、72.19%、74.23%和36.84%,消化性溃疡Hp感染率最高;胃黏膜炎症、肠上皮化生、低级别上皮内瘤变、高级别上皮内瘤变、GC的Hp感染率分别为52.43%、76.62%、55.04%、47.62%、36.84%,肠上皮化生组Hp感染率明显高于其他组,随着病变程度加深Hp感染率逐渐下降。结论中老年慢性胃病与HP感染密切相关,随着年龄增长,HP感染率呈下降趋势。HP感染在胃黏膜炎症、肠上皮化生、不典型增生、GC病理学改变、进展中起促进作用,应早期根除Hp。
[Abstract]:Objective to investigate the relationship between Helicobacter pylori (Hp) infection and pathological changes of gastric mucosa in middle-aged and aged patients with chronic gastric disease. Methods 631 patients over 50 years old with chronic gastropathy underwent gastroscopy, pathological biopsy and ~ (13) C-urea breath test (~ (13) C-UBT).
