白细胞介素-12B及其受体在炎症性肠病患者外周血和肠黏膜中的表达及意义

【摘要】：目的探讨白细胞介素(IL)-12B及其受体在炎症性肠病(IBD)患者外周血及肠黏膜中的表达和意义。方法通过结肠镜下收取结肠黏膜标本,其中活动期的克罗恩病(CD)患者、溃疡性结肠炎(UC)患者各60例,健康对照者50例。使用Western blot对活动期IBD患者发病部位肠黏膜蛋白水平进行检测,以同期结肠镜检查未见异常的患者作为对照。收集患者及健康体检者的外周血2 mL,留取血浆,提取血细胞总RNA,用Realtime PCR法检测IL-12B及其受体IL-12RB1的mRNA表达,用酶联免疫吸附试验(ELISA)法检测血浆IL-12p40的表达。结果外周血Real-time PCR结果显示IL-12B mRNA在CD、UC患者中的表达均增高,且差异有统计学意义(P0.05);IL-12RB1的mRNA在CD、UC患者中表达也增高,但只有CD患者有统计学意义(P0.05)。肠黏膜Real-time PCR结果显示IL-12B mRNA在CD、UC患者中的表达均增高,且差异有统计学意义(P0.05);而IL-12RB1 mRNA的表达在3组之间差异无统计学意义(P0.05)。外周血及肠黏膜的Realtime PCR结果显示IBD患者中Th细胞特异性转录因子(T-bet、GATA3、RORγ-t和Foxp3)的mRNA表达有不同程度的改变,且变化都有统计学意义。相关性分析显示,外周血IL-12B与T-bet的mRNA表达呈正相关(r=0.781,P0.01);肠黏膜IL-12B与T-bet和RORγ-t的mRNA表达均呈强正相关(r=0.807,P=0.000;r=0.831,P=0.000)。血清ELISA结果显示IL-12p40在IBD患者中的表达均高于健康对照者,且差异有统计学意义(P0.05)。CD患者IL-12p40表达高于UC患者(P0.05)。肠黏膜Western blot结果示IL-12p40在CD、UC患者中的表达均高于正常对照组,但只有CD患者IL-12p40蛋白的表达高于UC患者(P0.05),其受体IL-12RB1、IL-12RB2蛋白在CD、UC中表达也高于正常对照组,但差异无统计学意义(P0.05)。结论 IL-12B mRNA和蛋白在活动期CD患者外周血及病变部位肠黏膜中表达均明显上调。IL-12B可能通过Th1和Th17途径在IBD患者尤其是CD患者炎症过程中发挥重要的促炎作用。
[Abstract]:Objective to investigate the expression and significance of interleukin-12B (IL)-12B) and its receptor in peripheral blood and intestinal mucosa of patients with inflammatory bowel disease (IBD). Methods colonic mucosa specimens were collected under colonoscopy, including 60 patients with active Crohn's disease (CD), 60 patients with ulcerative colitis (UC) and 50 healthy controls. The intestinal mucosal protein levels in patients with active IBD were detected by Western blot, and those with no abnormal colonoscopy were used as controls. Plasma 2 mL, was collected from patients and healthy subjects. The total RNA, of blood cells was extracted and the mRNA expression of IL-12B and its receptor IL-12RB1 was detected by Realtime PCR method, and the expression of plasma IL-12p40 was detected by enzyme linked immunosorbent assay (ELISA). Results the results of peripheral blood Real-time PCR showed that the expression of IL-12B mRNA in patients with CD,UC was increased, and the difference was statistically significant (P0.05). The expression of IL-12RB1 mRNA was also increased in CD,UC patients, but only CD patients had statistical significance (P0.05). The results of Real-time PCR showed that the expression of IL-12B mRNA in patients with CD,UC increased significantly (P0.05), but there was no significant difference in the expression of IL-12RB1 mRNA among the three groups (P0.05). The results of Realtime PCR in peripheral blood and intestinal mucosa showed that the mRNA expression of Th cell-specific transcription factor (Th) in patients with IBD was changed to different degrees, and the changes were statistically significant (P < 0.01), but the expression of Gatt-3, ROR 纬-t and ROR 纬-t was significantly higher than that of the control group (P < 0.05). The correlation analysis showed that the expression of IL-12B in peripheral blood was positively correlated with mRNA expression of T-bet (r = 0.781, P0.01), and there was a strong positive correlation between IL-12B and mRNA expression of T-bet and ROR 纬-t in intestinal mucosa (r = 0.807, P < 0.001). (r = 0. 831, P < 0. 000). The results of serum ELISA showed that the expression of IL-12p40 in patients with IBD was significantly higher than that in healthy controls (P0.05). The expression of IL-12p40 in patients with CD was higher than that in patients with UC (P0.05). The results of Western blot showed that the expression of IL-12p40 in patients with CD,UC was higher than that in normal controls, but the expression of IL-12p40 protein in patients with CD was higher than that in patients with UC (P0.05), and its receptor IL-12RB1,IL-12RB2 protein was in CD,. The expression of UC was also higher in the control group than that in the control group, but there was no significant difference (P0.05). Conclusion the expression of IL-12B mRNA and protein is up-regulated in peripheral blood and intestinal mucosa of patients with active CD, IL-12B may play an important role in promoting inflammation in patients with IBD, especially in patients with CD through Th1 and Th17 pathway.
