食管胃静脉曲张精准内镜治疗方法的选择

【摘要】：目的:比较食管胃静脉曲张精准内镜治疗,在贲门上方治疗组、贲门下方治疗组、贲门上方和下方联合治疗组3组的疗效,观察血管消失及不良作用.方法:食管胃静脉曲张破裂出血患者104例,精准内镜治疗分为贲门上方治疗组68例,贲门下方治疗组15例,贲门上方和下方联合治疗组21例.比较3组静脉曲张治疗的总有效率、有效率、显效率、再出血率、并发症发生率.结果:治疗后3组静脉曲张总改善率、有效率、显效率,贲门上方组为63.2%、23.5%和39.7%,贲门下方组为80.0%、20.0%和60.0%,贲门上和下方联合治疗组90.5%、28.6%和61.9%.3组患者治疗后食管胃静脉曲张总有效率有统计学差异(?2=6.444,P=0.039),有效率、显效率有统计学差异(?2=6.235,P=0.044).治疗后2 wk和3 mo内再出血率,贲门上方治疗组为11.8%和19.1%,贲门下方治疗组为6.7%和13.3%,贲门上方和下方联合治疗组为9.5%和14.3%.3组再出血率均很低,但无统计学差异(P0.05).3组均无严重并发症发生,3组间并发症的发生率分别为30.9%、33.3%和38.1%,没有统计学差异(P0.05).结论:食管胃静脉曲张精准内镜治疗选择贲门上方和下方联合治疗疗效好.
[Abstract]:Objective: to compare the efficacy of accurate endoscopic treatment of esophageal and gastric varices in the treatment group above the cardia, the treatment group below the cardia and the combined treatment group above and below the cardia, and to observe the disappearance and adverse effects of blood vessels. Methods: 104 patients with esophageal and gastric variceal bleeding were divided into three groups: superior cardia treatment group (n = 68), inferior cardia treatment group (n = 15) and combined treatment group (n = 21). The total effective rate, rebleeding rate and complications rate of varicose veins in the three groups were compared. Results: after treatment, the total improvement rate, effective rate and effective rate of varicose veins in the three groups were 63.2%, 23.5% and 39.7%, 80.0%, 20.0% and 60.0%, respectively.
