广西汉、壮、瑶族7、10、13岁学生IGFs水平及生长发育影响因素的研究

【摘要】:[目的]通过对广西汉、壮、瑶族7岁、10岁、13岁学生的生长发育影响因素进行调查研究;了解广西汉、壮、瑶族儿童的血清胰岛素样生长因子-1(Insulin-like growth factor-1,IGF-1)、胰岛素样生长因子结合蛋白-3(Insulin-like growth factor binding protein-3,IGFBP-3)水平,不同民族、性别IGF-1、IGFBP-3水平存在的差异,汉、壮、瑶族7岁、10岁、13岁学生生长发育现状,生长发育水平存在的民族差异;探索影响儿童生长发育、IGF-1水平、IGFBP-3水平的因素以及胰岛素样生长因子(insulin-likegrowth factors,IGFs)对儿童生长发育的影响作用;为今后进一步开展研究、制定干预方案提供参考依据。 [方法]在广西合浦县、武鸣县、恭城县,分别抽取7岁、10岁和13岁的汉、壮、瑶族健康学生为研究对象。每个民族按各性别年龄组50人的标准抽样,,最终选取了946名学生,7岁组男生156人,女生155人;10岁组男生157人,女生156人;13岁组男生161人,女生161人。对他(她)们进行生长发育调查,测定其形态、机能、素质和性发育水平;在946名学生中以自愿为原则,抽取各性别年龄组30名学生作为IGFs和生长发育影响因素调查对象,检测标本598人份,剔除偏离较大的标本85人份,共占14.21%。实际采用检测数据513人份,汉、壮、瑶族学生各性别年龄组样本含量在22-35之间。 调查内容包括形态指标:身高、坐高、体重、胸围;机能素质指标:肺活量、立定跳远、斜身引体(7、10岁男生)或引体向上(13岁男生)、一分钟仰卧起坐(女生);性发育指标:男生睾丸体积、生殖器发育水平、女生乳房发育水平、阴毛发育水平;检测和统计方法:采用ELISA法检测汉、壮、瑶族学生IGF-1、IGFBP-3水平;对各项指标进行描述性分析,采用t检验、方差分析、秩和检验比较组间差异;分别以学生身高、体重为因变量,以父母身高、家庭经济状况、出生时体重、豆类及豆制品摄入情况、体育运动时间、睡眠质量、IGF-1水平、IGFBP-3水平等共30个指标为自变量进行多元线性逐步回归分析。 [结果] 1IGF-1、IGFBP-3: (1)IGF-1水平:7岁、10岁、13岁组男生依次为117.98μg/L、111.75μg/L、149.28μg/L,女生依次为125.14μg/L、108.63μg/L、146.25μg/L;血清IGF-1水平13岁组最高,7岁组次之,10岁组最低。7岁组学生血清IGF-1水平壮族最高;10岁组血清IGF-1水平汉族和瑶族学生差异没有统计学意义,壮族血清IGF-1水平最低;13岁组学生IGF-1水平壮族最高,瑶族次之,汉族最低。 (2)IGFBP-3水平:7岁、10岁、13岁组男生依次为62.42μg/L、61.74μg/L、61.27μg/L,女生依次为65.11μg/L、59.32μg/L、61.89μg/L;IGFBP-3水平年龄组间差异没有统计学意义。7岁组IGFBP-3水平瑶族和壮族学生差异没有统计学意义且均高于汉族学生;10岁组IGFBP-3水平瑶族学生最高,汉族最低;13岁组IGFBP-3水平汉族和瑶族学生差异没有统计学意义,且均高于壮族学生。 2生长发育 (1)形态指标:壮族形态指标发育水平均优于汉族和瑶族学生。身高:7岁、10岁、13岁组男生平均值依次为122.11cm、138.03cm、154.16cm,女生依次为121.72cm、138.21cm、152.71cm;坐高:7岁、10岁、13岁组男生平均值依次为66.96cm、73.85cm、81.41cm,女生依次为66.70cm、73.87cm、82.11cm;体重:7岁、10岁、13岁组男生平均值依次为23.99Kg、33.20Kg、42.31Kg,女生依次为22.42Kg、31.10Kg、42.53Kg;胸围:7岁、10岁、13岁组男生平均值依次为59.00cm、65.69cm、72.47cm,女生依次为57.49cm、64.63cm、73.50cm。 (2)机能素质指标:除13岁组男生引体向上、7岁组女生立定跳远、13岁组女生仰卧起坐不存在民族差异外,其余性别年龄组各项指标均存在民族差异。肺活量:7岁、10岁、13岁组男生平均值依次为1310.60ml、1957.39ml、2865.88ml,女生依次为1180.36ml、1776.34ml、2383.11ml;立定跳远:7岁、10岁、13岁组男生平均值依次为119.06cm、148.81cm、194.41cm,女生依次为112.26cm、140.60cm、165.07cm;斜身引体/引体向上:7岁、10岁、13岁组男生平均值依次为8.71次、15.31次、5.12次;仰卧起坐:7岁、10岁、13岁组女生平均值依次为18.32次、27.05次、28.65次。 (3)身体指数:身高坐高指数仅7岁组学生存在民族差异。BMI指数仅10岁组男生存在民族差异;10岁组汉、壮、瑶族男生BMI指数依次为16.30kg/m2、18.40kg/m2、17.03kg/m2。壮族学生超重和肥胖发生率较高。肺活量体重指数7岁和10岁组汉族学生高于壮、瑶族学生,13岁组壮族男生最高为73.41ml/kg。 (4)性发育情况:男生生殖器发育水平仅13岁组有民族的差异,睾丸发育分期10岁和13岁组有差异。男生睾丸体积汉族7岁、10岁、13岁组均值依次为2.66ml、5.06ml、16.00ml;壮族依次为2.23ml、5.80ml、19.03ml;瑶族依次为2.93、4.23、10.44ml。女生乳房和阴毛发育在10岁、13岁组有民族差异。性发育分期皆为瑶族学生落后。 3影响因素分析3.1IGF-1、IGFBP-3水平影响因素 (1)影响IGF-1水平的因素有血清IGFBP-3水平、身高、牛奶或豆浆摄入情况、午睡情况、母亲体重。 (2)影响IGFBP-3水平的因素有血清IGF-1水平、身高、牛奶或豆浆摄入情况、午睡情况、吃零食情况。 3.2生长发育影响因素 (1)影响身高发育的因素有IGF-1水平、IGFBP-3水平、家庭人口、经济水平、是否挑食、吃早餐情况、平时是否喝牛奶或豆浆、主食量、青菜摄入情况、最后一节课饥饿感、每天户外活动的时间、夜晚睡眠时间。 (2)影响体重发育的因素有IGF-1水平、IGFBP-3水平、家庭人口、父亲体重、出生体重、是否挑食、吃早餐情况、平时是否喝牛奶或豆浆、主食量、最后一节课饥饿感、午睡情况、夜晚睡眠时间。 [结论]身高、坐高、体重、胸围、肺活量壮族学生最优,立定跳远和仰卧起坐瑶族学生最优,斜身引体汉族学生最优,机能素质水平与形态发育水平不协调;IGF-1水平和IGFBP-3水平互为影响因素,影响IGF-1水平和IGFBP-3水平的因素除身高、牛奶或豆浆摄入情况、午睡情况外,母亲体重对IGF-1水平亦有影响,吃零食情况对IGFBP-3水平也有影响;身高、体重发育的影响因素除血清IGF-1、IGFBP-3水平外,营养、运动和睡眠状况亦包括其中,但民族因素没有入选。应进一步分析和验证血清IGF-1、IGFBP-3水平与生长发育的相关关系,血清IGF-1、IGFBP-3水平的影响因素;根据影响汉、壮、瑶族学生生长发育的因素采取措施促进汉、壮、瑶族学生体质均衡发展,
[Abstract]:[Objective] by Guangxi Han Zhuang, Yao, 7 years old, 10 years old, to investigate the influencing factors on the growth of 13 year olds; understand the Guangxi Han, Zhuang, Yao children's serum insulin-like growth factor -1 (Insulin-like growth factor-1, IGF-1), insulin-like growth factor binding protein -3 (Insulin-like growth factor binding protein-3, IGFBP-3) level, different nationalities, gender differences, IGF-1, IGFBP-3 level in the presence of the Han, Zhuang, Yao, 7 years old, 10 years old, 13 years of growth and development status of students, ethnic differences in level of growth and development; to explore the influence of children's growth and development, the level of IGF-1, factor IGFBP-3 levels and insulin like growth factor (insulin-likegrowth factors, IGFs) on the growth and development of children; for further study, to provide reference for the intervention.
[method] in Guangxi Hepu County, Wuming County, Gongcheng County, were selected at the age of 7, 10 and 13 year old Han, Zhuang, Yao students as the research object. According to the national standard sample of each sex age group 50, the final selection of 946 students, 7 year old group of 156 boys and girls 155 people; 10 years old group, 157 boys, 156 girls; 13 year old group of 161 boys and 161 girls. He (she) who investigated the growth, determination of the form, function, quality and development level; the 946 students voluntarily, from each sex age group 30 students as IGFs and growth factors survey, test specimens of 598 Cases, excluding the deviation specimens of 85 cases, accounted for 14.21%. the actual test data of 513 people, Han, Zhuang, Yao students in each gender and age group samples were between 22-35.
The survey includes morphological indicators: height, sitting height, body weight, chest circumference; function of quality indicators: vital capacity, standing long jump, pull (7,10 year old male) or chinning (13 boys), one minute sit ups (female); sexual development index: male testicular volume, genital development level, the development level of breast girls, pubic hair development level; detection and statistical methods: ELISA method was used to detect the Han, Zhuang, Yao students IGF-1, IGFBP-3 level; descriptive analysis of the indicators, using t test, analysis of variance, the difference between the groups with rank sum test; students' height, weight as the dependent variable, with the height of parents, family economic the status, birth weight, beans and bean products intake, physical exercise, sleep quality, IGF-1 level, IGFBP-3 level of a total of 30 indicators as independent variables stepwise multiple linear regression analysis.
[results]
1IGF-1, IGFBP-3:
The level of IGF-1 (1): 7 years old, 10 years old, 13 years old group of boys were 117.98 g/L, 111.75 g/L, 149.28 g/L, girls were 125.14 g/L, 108.63 g/L, 146.25 g/L; serum IGF-1 level is the highest 13 years old group, 7 years old group, 10 years old.7 years old was the lowest. The serum level of IGF-1 group of students at the age of 10 the highest Zhuang; levels of IGF-1 in Han and Yao students there is no significant difference in serum IGF-1 level, minimum 13 years old group of students of Zhuang nationality; the IGF-1 level of the highest Zhuang, Yao, Han minimum.
The level of IGFBP-3 (2): 7 years old, 10 years old, 13 years old group of boys were 62.42 g/L, 61.74 g/L, 61.27 g/L, girls were 65.11 g/L, 59.32 g/L, 61.89 g/L; differences in the level of IGFBP-3 between age groups was not statistically significant.7 age group IGFBP-3 Yao and Zhuang Students differences no statistically significant and were higher than Han students; minimum 10 years old group IGFBP-3 Han Yao students; the highest level, 13 year old group IGFBP-3 level of Han and Yao students the difference was not statistically significant, and higher than that of students of Zhuang nationality.
2 growth and development
(1) morphological index: Zhuang morphological development indicators were better than those of Han and Yao students. Height: 7 years old, 10 years old, 13 year old male group average value were 122.11cm, 138.03cm, 154.16cm, 138.21cm, 121.72cm were female, 152.71cm; sitting height: 7 years old, 10 years old, 13 year old male group mean life the order of 66.96cm, 73.85cm, 81.41cm, 73.87cm, 66.70cm were female, 82.11cm; weight: 7 years old, 10 years old, 13 year old male group average value were 23.99Kg, 33.20Kg, 42.31Kg, 31.10Kg, 22.42Kg were female, 42.53Kg; chest: 7 years old, 10 years old, 13 years old group of boys are average 59.00cm, 65.69cm, 72.47cm, 64.63cm, 57.49cm were female, 73.50cm.
(2) the function of quality index: in 13 years old group of boys pull-up, 7 year old girl standing long jump, 13 groups of girls sit ups does not exist ethnic differences, the rest of the index of sex and age groups were ethnic differences. Capacity: 7 years old, 10 years old, 13 year old male group average value were 1310.60ml, 1957.39ml girls, 2865.88ml, followed by 1180.36ml, 1776.34ml, 2383.11ml; standing long jump: 7 years old, 10 years old, 13 year old male group average value were 119.06cm, 148.81cm, 194.41cm, 140.60cm, 112.26cm were female, 165.07cm; pull / pull ups: 7 years old, 10 years old, 13 year old boys group average in order for 8.71 times, 15.31 times, 5.12 times; sit ups: 7 years old, 10 years old, 13 year old girl group average were 18.32 times, 27.05 times, 28.65 times.
(3) the body index: sitting height index is only 7 years old group of students from different nationalities.BMI index are different between only 10 boys; 10 year old group of Han, Zhuang, Yao boys BMI index were 16.30kg/m2,18.40kg/m2,17.03kg/m2. students of Zhuang Nationality in the incidence of overweight and obesity is higher. Lung weight index of 7 and 10 years old Han group the student is higher than that of Zhuang, Yao students, 13 year old Zhuang group was the highest 73.41ml/kg. boys
(4) sexual development: male genital development level of ethnic differences is only 13 years old group, 10 stage and 13 year old group differences in testicular development. Male testicular volume 10, Han 7 years old, mean 13 years old group were 2.66ml, 5.06ml, 16.00ml; Zhuang were 2.23ml, 5.80ml, 19.03ml; Yao in order for the 2.93,4.23,10.44ml. female breast and pubic hair development in 10 years, 13 year old group of ethnic differences. Development stages are Yao students behind.
3 influencing factors analysis 3.1IGF-1, IGFBP-3 level influence factors
(1) the factors affecting the IGF-1 level were serum IGFBP-3 level, height, milk or soybean milk intake, nap, mother weight.
(2) the factors affecting the IGFBP-3 level were serum IGF-1 level, height, milk or soybean milk intake, nap situation, and snack condition.
3.2 influence factors of growth and development
(1) the factors that affect height development include IGF-1 level, IGFBP-3 level, family population, economic level, whether picky food, breakfast, milk or soybean milk, main food intake, greens intake, last class hunger, daily outdoor activities and night sleep time.
(2) the factors that affect weight development include IGF-1 level, IGFBP-3 level, family weight, father weight, birth weight, whether to be choosy, breakfast, milk or soy milk, main food, hunger, nap and sleep time.
[Conclusion] the height, sitting height, body weight, chest circumference, vital capacity of Zhuang Students best, standing long jump and sit up Yao students optimal pull Han students optimal function level and morphological development level is not coordinated; IGF-1 level and IGFBP-3 level are influencing factors, the influence factors of the IGF-1 and IGFBP-3 level in addition to height, milk or Soybean Milk intake, napping, maternal weight also has influence on the level of IGF-1, eat snacks also have influence on the level of IGFBP-3; influence factors of height, weight development in serum IGF-1, IGFBP-3 level, nutrition, exercise and sleep status also included, but not included in national factors should be further analyzed and verified. Correlation between serum IGF-1, IGFBP-3 levels and growth of serum IGF-1, the effect factors of IGFBP-3; according to the influence of Han, Zhuang, Yao students growth factors to take measures To promote the balanced development of the physique of the Han, Zhuang and Yao students.
