焦炉工外周血淋巴细胞微核率与尿中多环芳烃代谢物、血浆BPDE-白蛋白加合物的关联及其在工人早期健康监护中的应用

【摘要】:目的:全面评价焦炉工尿中10种羟基多环芳烃(monohydroxylated PAHs,OH-PAHs)、血浆二氢二醇环氧苯并芘(Benzo[a]pyrene-diol-epoxide,BPDE)-白蛋白加合物水平与外周血淋巴细胞微核率的关联和探索能否找到一个合适的微核率截断值用于早期监测工人的健康状态。 方法:选取某焦化厂1315名男性工人作为研究对象,对该厂代表性工作场所空气中多环芳烃(Polycyclic aromatic hydrocarbons,,PAHs)的浓度进行了测定。分别用气相色谱-质谱联用仪和酶联免疫吸附实验测定工人尿中10种OH-PAHs和血浆BPDE-白蛋白加合物的浓度,用高效液相色谱测定工人尿中8-羟基鸟苷(8-hydroxy-2’-deoxyguanosine,8-OHdG)的浓度,用胞质分裂阻滞法测定工人外周血淋巴细胞微核率。 结果:校正了年龄、工龄、吸烟状态、饮酒状态的影响后,工人尿1-羟基萘、2-羟基萘、2-羟基芴、9-羟基芴(9-hydroxyfluorene,9-OHFLU)、1-羟基菲、2-羟基菲、3-羟基菲、9-羟基菲(9-hydroxyphenanthrene,9-OHPHE)、1-羟基芘(1-hydroypyrene,1-OHP)、血浆BPDE-白蛋白加合物的水平与外周血淋巴细胞微核率的关联具有显著性(P值分别为0.001、0.002、0.001、0.001、0.016、0.001、0.003、0.001、0.001、0.001)。当校正了PAHs代谢物间相关的影响后,工人尿1-OHP、9-OHFLU、血浆BPDE-白蛋白加合物的水平与微核率间的关联依然具有显著性(经过多重比较校正,P值0.0056)。与微核率≤2‰的工人相比,微核率≥5‰的工人尿总OH-PAHs、1-OHP、9-OHFLU、9-OHPHE、8-OHdG和血浆BPDE-白蛋白加合物的水平开始出现显著增高。趋势分析表明在校正了混杂因素的影响后,工人尿总OH-PAHs、1-OHP、9-OHFLU、9-OHPHE、8-OHdG和血浆BPDE-白蛋白加合物的水平从低微核率组到高微核率组呈现显著升高的趋势(P值分别为0.017、0.005、0.002、0.015、0.015、0.001)。 结论:焦炉工尿中除了4-羟基菲外的其余9种OH-PAHs和血浆BPDE-白蛋白加合物的水平与外周血淋巴细胞微核率的关联均具有显著性。尿1-OHP、9-OHFLU、血浆BPDE-白蛋白加合物与微核率升高间的关联具有显著性。可以用微核率≥5‰对焦炉工进行早期健康监护。
[Abstract]:Objective: to evaluate the relationship between the levels of 10 monohydroxylated PAHs OH-PAHs, plasma dihydrodiol epoxide Benzo [a] pyrene-diol-epoxide-albumin adducts and the micronucleus rate of peripheral blood lymphocytes in coke oven workers and to explore whether a suitable micronucleus can be found. The rate truncation value is used to monitor the health status of workers at an early stage. Methods: the concentrations of polycyclic aromatic hydrocarbonsPAHs in the air of typical workplace of a coking plant were determined in 1315 male workers. The concentrations of 10 kinds of OH-PAHs and plasma BPDE- albumin adducts in urine of workers were determined by gas chromatography-mass spectrometry and enzyme-linked immunosorbent assay, respectively. The concentrations of 8-hydroxy-2-hydroxy-deoxyguanosine 8-OHdG in urine of workers were determined by high performance liquid chromatography. The micronucleus rate of peripheral blood lymphocytes of workers was determined by cytokinesis block method. Results: after adjusting the effects of age, length of service, smoking status and drinking state, Urinary 1-hydroxynaphthalene 2-hydroxy-naphthalene 2-hydroxy-2-hydroxyfluorofluorene 9-hydroxyfluorenene-9-OHFLUU 1-hydroxyphenanthroline 2-hydroxy-phenanthroline 3-hydroxyphenanthroline 9-hydroxyphenanthrene9-OHPHEHEN9-hydroxy 1-hydroyrerenenepyrene 1-OHPPPU, the levels of plasma BPDE-albumin adducts were significantly correlated with the micronucleus rate of peripheral blood lymphocytes (P = 0.0010.0020.0020.0010.001P = 0.0010.001P = 0.001P = 0.001P = 0.001P = 0.001P = 0.001P = 0.001P = 0.001P = 0.001P = 0.001P = 0.001P = 0.001P = 0.001P = 0.001P = 0.001P = 0.001P = 0.001P = 0.001P = 0.001P = 0.001P = 0.001P = 0.001P = 0.001P = 0.001. After adjusting for the correlation between PAHs metabolites, the correlation between the levels of plasma BPDE-albumin adducts and micronucleus rate in the urine of workers with 1-OHP- 9-OHFLU was still significant (P = 0.0056).
