中国西部城市孕妇末梢血矿物质元素检测水平分析及建议参考值研究

【摘要】:目的 分析中国西部三省份3339例18-42岁妊娠中期孕妇末梢血钙、铁、锌、铜、镁五种矿物质元素含量的检测结果,了解上述五种矿物质元素在该人群中缺乏和过量现状及其原因,分析五种矿物质元素在地区、年龄之间的差异,确立孕妇末梢血五种元素参考范围值。 方法 采取整群随机抽样的方法选取中国西部甘肃、陕西、四川三省18-42岁定期做孕期检查、资料完整的妊娠中期健康孕妇3339例为样本。使用多通道火焰原子吸收光谱仪(BH5100型)检测孕妇末梢血中钙、铁、锌、铜、镁五种元素含量,对各省份各年龄组孕妇末梢血五种元素含量对比分析,最后用百分位数法制定五种元素参考范围值。 结果 1.研究样本的基本情况:本研究样本来自甘肃、陕西、四川三省份,共3339例,平均年龄27.09±4.44岁,其中甘肃和陕西25-岁组例数最多,而四川省18-岁组最多,各省孕妇年龄分布符合各省的一般特征,代表性较好。 2.三省孕妇钙、铁、锌、铜、镁元素现状:三省份孕妇钙元素含量均值在正常范围值之内,甘肃省含量最低,陕西省最高,不同年龄组中30-岁组最高,且孕妇钙元素含量有随孕妇年龄最高而增高的趋势,三省份孕妇钙缺乏较为严重,其中陕西省缺乏率最低为25.94%,甘肃省缺乏率最高甚至达到了53.18%,孕妇钙过量现象也存在:孕妇铁元素均值在正常范围之内,三省份含量差异具有统计学意义(P0.05),其中四川省最低,甘肃省最高,不同年龄组中25-岁组最高,孕妇铁缺乏严重,甘肃省缺乏率最低8.47%,四川省缺乏率最高59.30%,铁过量的现象只在甘肃省出现;锌元素含量均值在正常范围之内,三省份含量差异具有统计学意义(P0.05),其中四川最低,甘肃最高,不同年龄组中25-岁组最高,孕妇锌缺乏现象较为普遍,甘肃缺乏率5.10%,四川缺乏率最高13.99%,锌过量例数较少,不普遍;铜元素含量均值在正常范围之内,三省份均值有统计学差异(P0.05),甘肃省最低,四川省最高,不同年龄组中18-岁组最高,铜缺乏率四川最低0.10%,甘肃最高16.61%,铜过量现象存在但比较罕见;镁元素含量均值在正常范围之内,四川省均值最低,甘肃最高,不同年龄组中25-岁组最高,镁缺乏和过量现象均不普遍。 3.末梢血五种元素参考值范围的确立:按照不同省份、不同年龄组(18-岁组、25-岁组、30-岁组)确立末梢血钙、铁、锌、铜、镁元素参考值范围,分别取第2.5、10、16、25、50、75、84、90、97.5百分位数值,确立期望参考值范围(P16~P84%)和临界参考值范围(P2.5~P97.5),为临床检测提供参考依据。 结论 1.三省份妊娠中期孕妇末梢血中钙、铁、铜、镁元素含量因自然环境、气候条件、膳食结构不同而表现出地区差异,但三省份各年龄组孕妇各元素含量平均值均在正常参考值范围内。 2.钙、铁、锌元素三省份各年龄组孕妇缺乏均较为严重,铜、镁元素缺乏较少。 3.孕妇末梢血中钙、铁、铜、镁元素过量现象均存在,但仅限于个别孕妇。 4.确立三省份各年龄组末梢血钙、铁、铜、镁元素参考值范围,为临床诊断提供参考依据。
[Abstract]:Purpose
The results of the determination of five mineral elements in maternal peripheral blood calcium , iron , zinc , copper and magnesium in 3339 pregnant women aged 18 - 42 in western China were analyzed .
method
Three hundred and thirty - nine children aged 18 - 42 in western Gansu , Shaanxi and Sichuan were randomly sampled by cluster random sampling . The contents of Ca , Fe , Zn , Cu and Mg in the peripheral blood of pregnant women were analyzed by multi - channel flame atomic absorption spectrometer ( BH5100 type ) .
Results
1 . The basic information of study samples : The samples from Gansu , Shaanxi and Sichuan were 3339 cases , the average age was 27.09 卤 4.44 years . Among them , the age distribution of pregnant women in Gansu and Shaanxi was the largest , and the age distribution of pregnant women in every province was in accordance with the general characteristics of the provinces .
2 . Current status of calcium , iron , zinc , copper and magnesium in pregnant women in three provinces is the lowest in the three provinces , the highest in Gansu Province , the highest in Shaanxi Province , the highest in Shaanxi Province , the highest in Shanxi Province , and the highest in the three provinces .

The mean value of copper element was within the normal range , the mean of three provinces had statistical significance ( P0.05 ) , the lowest in Gansu Province , the highest in Sichuan Province , the highest in the 18 - year - old group in different age groups , the lowest copper deficiency rate in Sichuan was 0.10 % , the highest in Gansu was 16.61 % , and the copper overdose was rare ;
The mean value of magnesium element is within the normal range , the lowest in Sichuan province , the highest in Gansu , the highest in the age group of 25 - year old in different age groups , and the magnesium deficiency and excessive phenomenon are not common .
3 . The reference value range of the five elements of peripheral blood was established : according to different provinces , different age groups ( aged 18 - year group , 25 - year group , 30 - year group ) established the reference value range of peripheral blood calcium , iron , zinc , copper and magnesium element . The reference value range ( P16 ~ P84 % ) and critical reference value range ( P2.5 ~ P97.5 ) were established , and the reference basis was provided for clinical detection .
Conclusion
1 . The contents of calcium , iron , copper and magnesium in the peripheral blood of pregnant women in three provinces were different from those of the natural environment , climatic conditions and dietary structure , but the average values of all the pregnant women in each age group were within the normal reference value range .
2 . The deficiency of pregnant women in all age groups in the three provinces of calcium , iron and zinc is more serious , and the deficiency of copper and magnesium is less .
3 . The excessive phenomenon of calcium , iron , copper and magnesium in the peripheral blood of pregnant women exists , but it is limited to individual pregnant women .
4 . To establish the reference value range of blood calcium , iron , copper and magnesium in each age group in three provinces and provide reference for clinical diagnosis .
